摘 要:目的探讨HIV感染者血清β2-微球蛋白的临床意义。方法检测HIV感染者血β2-微球蛋白、CD4^+T淋巴细胞计数、HIV病毒载量、尿素氮和肌酐,分析其血清β2-微球蛋白浓度变化及与病毒载量、CD4^+T淋巴细胞计数的相关性以及肾功能的变化。结果HIV感染组血清β2-微球蛋白浓度明显高于正常对照组(P〈0.05),且A,B,C三期逐渐升高(P〈0.05),B,C两期患者明显高于A期(P〈0.05),B期与C期比较差异无统计学显著性意义(P〉0.05);AIDS组血清β2-微球蛋白浓度显著高于非AIDS组(P〈0.05);死亡组显著高于未死亡组(P〈0.05);24例患者接受HARRT治疗三月后血清β2-微球蛋白浓度显著降低(P〈0.05)。HIV感染者血β2-微球蛋白浓度与CD4^+T淋巴细胞计数呈负相关(r=-0.419,P〈0.01),与病毒载量呈正相关(r=0.571,P〈0.01)。几乎所有的HIV感染者肾功能指标都正常。结论HIV感染者血清β2-微球蛋白升高,可能与免疫系统被激活和淋巴细胞被破坏致机体产生了更多的β2-微球蛋白有关,可反映患者体内病毒复制情况。体内高浓度的β2-微球蛋白可能提示患者预后不佳。血清β2-微球蛋白浓度的检测是进行临床分期、病情预测的重要指标,并可评价HARRT疗效。Objective To investigate the clinical significance of N microglobulin in sera of HIV/AIDS patients. Methods Detected the levels of β2-microglobulin(β2-MG),Cr,BUN and HIV-RNA vires loads in sera of HIV/AIDS patients,and T lymphocyte subsets count in whole blood and analyzed the correlations between them. Results The level of β2-MG in serum of HIV/AIDS patients was significantly higher than that in healthy group (P〈0.05). Stadge B and C was significantly higher than that in stadge A,but stadge B was no significance than that in stadge C. Serum β2-MG in AIDS patients was significantly higher than that in none-AIDS patients. Similarly,the death group was significantly higher than that in lively patients. β2-MG in blood was positively correlative with HIV-RNA ,while negatively correlative with CD4 count. And kidney function of almost all patients in this observation were normal. Conclusion The majority of HIV/AIDS patients β2-MG level in blood were elevated and it would be relatively higher with disease progression to AIDS phase. These observations above indicate that the increase of β2-MG in blood may be relative to the destruction of lymphocyte and activation of immune system in HIV/AIDS infection,and is probably due to more β2-MG produced by the individuals,but not the decrease of glomerular filtration rate (GFR) of patients' kidney.
