摘  要:目的;研究丙肝病毒基因型与丙肝患者使用IFN-α2b治疗效果的关系。方法;采用HCV基因组NS5区PCR产物酶切分型方法,对79例丙肝患者做HCV基因分型。所有病例均按预定方案应用IFN-α2b治疗,根据ALT的变化情况和判断疗效。首先考察应用1MU和3MU IFN-α2b两种剂量在治疗效果上的差异性;然后研究HCV基因型对IFN-α2b治疗效果的影响。Objective: To study the relationship between HCV genotypes and the curative effect of interferon - alpha 2b(IFN - α2b) in tke patients suffered from hepatitis C. Methods: 79 patient suffered from HCV were classified with the restriction endonuclease technique of polymerase chain reaction (PCR) product of HCV genome non - structural region 5(NS5). All the patients were treated according to predetermined course. The curative effects were evaluated by the changes of alamine aminotransferase(ALT). Firstly, the difference of therapy efficacy was tested between 1MU and 3MU IFN - α2b. Then, the effect of HCV genotypes on IFN - α2b efficacy was investigated. Result: Of the 79 patients, 45 fell under type Ⅱ of HCV(57%), 28 type Ⅲ(35%). and 6 belonged to type Ⅱ/Ⅲ co -infection(8%).The curative effect was studied after the course of treatment. The results indicated that the curative effect was not significantly different(P>0.5) between IFN - α2b 1M and IFN - α2b 3MU. Of the 45 patients with HCV type Ⅱ, 9 were complete responders(CR), 14 patial responders(PR), and 22 non -responders(NR). However, of 28 patients with HCV type Ⅲ 16 were CR, 7 PR and 5 NR. It was proved that the therapy efficacy in type Ⅲ was evidently better than that in type Ⅱ (P<0.01). Conclusion: When the patients suffered from HCV were treated with IFN - α2b, the dosage should not be blindly in creased to improve curative effect. On the contrary, more attention should be paid to the effect of HCV genotypes, whick may be an important indxe of predicting curatiev effect.
