摘　　要：目的了解农村老年人群血脂现况并进行危险性评估。方法采用整群随机抽样的方法,对1975名清河县70岁以上老年人进行问卷调查、体格检查和血生化检测。血脂水平和危险分层标准参照2007年《中国成人血脂异常防治指南》。结果该人群血脂异常检出率63.1%,男性52.3%,女性73.6%,差别有统计学意义(P<0.01)。总胆固醇或低密度脂蛋白胆固醇异常(包括边缘升高和升高)的1002人中,处于高危和中危组的比例分别为55%和33%。血脂正常、血脂边缘升高及高脂血症人群收缩压均值分别为153.2mm Hg、158.0mm Hg和158.6mm Hg(P<0.001),舒张压均值分别为84.0mm Hg、85.7mm Hg、86.0mm Hg(P=0.009);高血压检出率分别为69.5%,76.3%和77.8%(P<0.001)。结论清河县农村老年人群血脂异常率高,危险性评估显示高危组比例高。血脂水平与血压水平、高血压检出率关系密切。Objective To explore the distribution of blood lipids and assess its risk status in Chinese rural elderly population. Methods With cluster sampling method, 1 975 farmers （men 971, women 1004） aged 70-90 years old finished all examinations, including questionnaire, physical check and blood test, and all data was analyzed. The cut points and risk stratification of dyslipidemia were categorized according to the ＂Chinese Adults Guideline for Prevention and treatment of dyslipidemia in 2007＂. Results The total proportion of abnormal blood lipids in this elderly population was 63.1% （52.3% in male, 73.6% in female, P〈 0.01）. The risk assessment in the population with abnormal TC or LDL-C （1 002 persons） was stratified into high （55%）, moderate （33%） risk subgroup, respectively. In normal blood lipids group, borderline hyperlipidemia group and hyperlipidemia group, the mean of SBP was 153.2, 158.0 and 158.6 mmHg, respectively （P〈0.001） ; the mean of DBP was 84.0, 85.7 and 86.0 mmHg respectively （P〈0.01）; the prevalence of hypertension was 69.5%, 76.3% and 77.8%, respectively （P〈0.001）. Conclusion Our results suggest that the prevalence of dyslipidemia in the rural elderly is high in recent years, and a half of them are in the high risk category. Abnormal lipids and blood pressure level were closely related in Chinese rural elderly population.
