摘  要:目的:评价椎弓根钉系统内固定并经椎弓根植入定量自体松质骨和骨髓治疗胸腰椎爆裂性骨折的临床效果。方法:对35例胸腰椎爆裂性骨折患者实施后路撑开复位内固定或加经椎弓根移植自体骨和骨髓进行治疗。对比观察不同时期伤椎的前后缘高度比、楔变角、后凸角的变化。结果:35例病人术后早期椎体高度和形态得到明显恢复,Cobb’s角、伤椎前后缘相对高度也得到改善,但远期效果二组有明显差别,定量植骨组矫正角度丢失及高度丧失比未植骨组少。结论:后路椎弓根钉系统固定并经椎弓根定量移植自体骨和骨髓能增加远期椎体骨容量和脊柱前柱的抗压稳定性。Objective: To investigate the effect for treatment of thoracolumbar vertebral burst fractures by the method of pedicle screw fixation the vertebral body via pedicle with quantify autogenous bone and marrowbone. Method: 35 cases of thoracolumbar vertebral burst fractures were treated by pedicle screw fixation of vertebral body via pedicle or combining with autogenous bone and marrowbone. control study the index of RAP and AWA and Cobb' s angel preoperative to postoperative. Result: In a total of 35 cases, The height and configuration of centrum came back obviously and the Cobb' s angle improved in early period. But the late result is different. In group of the bone graft, The height and angle loss is less. Conclusion: Application of pedicle screw fixation of vertebral body transpedicular combining autogenous bone and marrowbone grafting can reconstruct vertebral shape and height and increase the stability of the spine.
