山东省部分地区已婚育龄妇女人工流产风险因素的病例-对照研究

摘　　要：目的:探讨山东省已婚育龄妇女人工流产发生的危险因素。方法:在4个县市采用随机整群抽样方法,对2005年1月1日～2006年6月30日接受人工流产的1 146例和未行流产的1 094例已婚育龄妇女进行结构式问卷调查,用SPSS15.0进行单因素与多因素Logistic回归分析,以OR值和95%CI作为评价指标。结果:在单因素分析中,与人工流产有关的因素有文化程度、避孕方法、人工流产史、引产史、丈夫对避孕的重视程度、发放药具是否及时、有无随访等;多因素分析显示,控制文化程度因素后,以前有人工流产和引产史、未采用宫内节育器(IUD)、使用短效避孕药具等因素仍然增加意外妊娠和流产的危险性。结论:应加强避孕方法知识的宣传及行为干预,提倡以长效避孕方法(如IUD)为主,降低妇女意外妊娠导致人工流产的风险。Objective： To explore the risk factors for induced abortion among the married women of reproductive age in Shandong.Methods： A stratified random sampling method was used to recruit 1 146 women who underwent induced abortion（the study group） on January 1,2005 to June 30,2006 and 1 094 married women of reproductive age（the control group） in 4 counties/cities of Shandong.A questionnaire survey was carried out in a face to face interview.The univariate and multivariate logistic regression analyses were conducted and odds ratios（ORs） and 95% confidence interval（CI） were calculated.Results： In the univariate analysis,the factors including educational levels,methods of contraception,induced abortion history in early-and mid-pregnancy,the attitude of husband to reliable contraception,timely delivery of contraceptive supplies and follow-ups had statistically significant relation to the occurrence of induced abortion.After the factor of educational level was controlled,other factors mentioned above were still related to the occurrence of unwanted pregnancy and induced abortion significantly.Conclusion： The propaganda and behavior intervention should be strengthened and long-term contraception should be promoted to reduce the incidence of induced abortion due to unwanted pregnancy.
