摘  要:目的通过监测新生儿缺氧缺血性脑病(HIE)患儿血清白介素-18(IL-18)和钙离子(Ca2+)的水平变化,探讨IL-18、Ca2+与HIE的相互关系及临床意义。方法HIE新生儿为HIE组,对照组为正常新生儿。2组分别于生后第1、3、7、14天测定血清IL-18和Ca2+,分析IL-18、Ca2+的变化及意义。结果HIE患儿生后第1、3、7天血清IL-18含量较对照组均有升高,Ca2+含量较对照组均有降低(P<0.01);且中、重度HIE患儿血清IL-18含量高于轻度HIE患儿,Ca2+含量低于轻度HIE患儿(P<0.01)。第14天HIE组的IL-18、Ca2+含量与对照组比较差异无统计学意义(P>0.05)。结论IL-18、Ca2+参与HIE的整个发病过程,检测血清中IL-18、Ca2+水平对判断HIE脑损伤的程度、病情发展及预后具有临床意义。Objective To observe the changes of levels of IL-18 and Ca^2+ in neonatal hypoxic-ischemic encephalopathy(HIE) ,and to investigate the correlation among IL-18, Ca^2+ and HIE. Methods 60 HIE patients were randomly classified. 10 normal neonates were enroILed as control group. In each group, the concentrations of IL-18 and Ca^2+ on 1 day,3 days,7 days and 14 days after birth. The the changes and significances of IL-18 and Ca^2+ were observed and analyszed. Results The levels of serum IL-18 in HIE groups on the first day after birth were significantly higher than those of the control group, however, the levels of Ca^2+ were significantly lower( P 〈 0. 01 ). The levels of IL-18 in patients with moderate and severe HIE were significantly higher than those of patients with mild HIE, while the levels of Ca^2+ were obviously lower( P 〈 0.01 ). There was no significant difference in the serum levels of IL-18 and Ca^2+ on the 14 days after birth between HIE groups and control group( P 〉0.05), Conclusion IL-18 and Ca^2+ are involved in the course of HIE. The levels of IL-18 and Ca^2+ in HIE patients are positively correlated with the severity, condition and prognosis of HIE.
