摘　　要：目的:探讨肾血管平滑肌脂肪瘤(renal angiomyolipoma,RAML)的CT征象及鉴别诊断。方法:回顾性分析26例行CT平扫及增强扫描并经手术病理证实为RAML的临床及CT资料,扫描设备为SOMATON SPIRIT双层螺旋CT扫描仪。结果:RAML的CT表现为圆形或类圆形,密度不均匀,有软组织及脂肪等多种成分或以某种成分为主的肿块。2例巨大肿瘤病灶合并出血。增强扫描表现为病灶不同程度强化,脂肪成分不强化,部分病例血管呈条索状和斑点状强化影。结论:典型RAML有其特征性的CT征象,病灶内是否含有脂肪组织是确诊的重要依据,增强扫描肿块内可见条索状斑点状强化影,颇具特征性。巨大肿瘤病灶内可合并出血。Objective：To study the CT signs and differential diagnosis of renal angiomyolipoma（RAML）.Methods：Clinical and CT data of 26 cases of RAML undergoing plain and contrast enhanced CT scan and confirmed by surgery and pathology were retrospectively analyzed.Results：CT findings of RAML showed that masses were of round or oval and uneven density,and that some were composed of many ingredients or mainly of one ingredient including soft tissue and fat.2 cases of huge tumor were complicated with bleeding lesions.Enhanced scan showed that lesions were variently enhanced,fat component was not enhanced,and that some patients＇ vessels presented strip-shaped and funicular,and spot-like enhanced image.Conclusion：Typical RAML has its unique CT signs of lesions.Whether the lesions contain fat tissue or not is an important basis for diagnosis.
