摘 要:[目的]了解高尿酸血症的危险因素,为制定高尿酸血症预防措施提供依据。[方法]采用1︰1配对的病例对照研究设计,所有病例均来自2008年2-12月在山东省随机选取的5个市的某医院内确诊的痛风及高尿酸血症患者,同时,以同一时间在该医院内科就诊的其他疾病患者为对照进行问卷调查并展开分析。[结果]经多因素条件Logistic回归分析,文化程度(OR=1.117)、个人月收入(OR=1.599)、平均喝啤酒量(OR=1.089)、经常饮茶(OR=1.299)、经常吃蛤蜊(OR=2.127)、经常锻炼身体(OR=0.917)、血尿酸(OR=8.187)等7个因素为高尿酸血症的主要危险因素,根据筛选出的7个危险因素建立"最优"模型,同时计算吃海鲜对高尿酸血症的人群归因分值22%。[结论]应用条件Logistic回归找出了高尿酸血症的危险因素,同时还根据各危险因素建立了相应的"最优"模型,达到了研究目的,为同类研究提供了可行的方法学上的参考。[Objective] To analyze the risk factors related to Hyperuricemia, and provide scientific evidence to health care policy maker for the control of Hyperuricemia. [Methods] The research subject came from the hospital of 5 cities in Shandong province from February 2008 to December 2008, suffering from gout and hyperuricemia and other disease patients who were planning to treat the internal medicine of hospital in the identical time. Experimental group and control group were designed according to age, nation, birth area and so on. We surveyed them and carried them out. [Results] Conditional logistic multiple regression analysis was carried out and showed that education (OR = 1.117), personal monthly income (OR = 1.599), the average volume of drinking beer (OR = 1.089), frequently drinking tea (OR = 1.299), often eat clams (OR = 2.127), regu- lar physical exercise (OR = 0.917) and serum uric acid (OR = 8.187). According to 7 dangerous factors we got, we finally discovered the finally ‘best’ model of this research. At the same time, we calculated the AFp of eating seafood for Hyperuricemia, it was 22%. [Conclusion] Risk factors of hyperuricemia were found out by application condition Logistic regression, and relative importance among various dangerous factors had been discovered through the quantitative analysis; and then we established the ‘best’ model and achieved the research goal, provided in the feasible methodology reference for the similar re- search.
