精卵孵育时间对体外受精-胚胎移植结局的影响

摘　　要：[目的]比较体外受精-胚胎移植中精卵孵育不同时间(4～6h和16～18h)对体外受精-胚胎移植结局的影响。[方法]272例体外受精-胚胎移植(IVF-ET)周期根据精卵孵育不同时间分为两组:A组:199例采集的卵子,在加精后4～6h取出放入新鲜培养液中培养;B组:73例采集的卵子,加精过夜培养后(16～18h)取出放入同种培养环境中培养。比较两组的受精率(包括正常受精率和异常受精率)、卵裂率、优质胚胎率、临床妊娠率以及胚胎种植率。[结果]A组受精率明显高于B组,差异有统计学意义(P﹤0.01);A组正常受精率及优胚率高于B组,差异有统计学意义(P﹤0.05);两组的卵裂率、临床妊娠率及胚胎种植率的差异均无统计学意义。[结论]缩短精卵孵育时间能够提高正常受精率和胚胎的质量,从而提高胚胎的种植潜能。[Objective] To compare the effect of different incubation time on the outcome of in vitro fertilization and em- bryo transfer （IVF-ET）. [Methods] A total of 272 women receiving IVF-ET treatment were included and divided into group A （199 cases） and group B （73 cases）. In group A, the oocytes were incubated with the sperm for 4～6 hours, then the oocytes were washed and transferred into the fresh medium. In group B, the oocytes were co-incubated with sperms for 16～18 hours. The fertility rates, cleavage rates, hight quality embryo rates, clinical pregnancy rates and implantation rates were compared between the two groups. [Results] The fertility rate of group A was significantly higher than that of group B （P﹤0.01）; The normal fertility rate and better embryo rate of group A was significantly higher than those of group B （P﹤0.05）, There was no significant difference between the two groups in cleavage rate, pregnancy rates and implantation rates. [Conclusion] The short time incubation of oocytes and sperm could improve the normal fertility rate and the quality of embryo so that to improve the implantation potency.
