背驮式原位肝移植的手术技术

摘　　要：为１例原发性肝癌患者作了背驮式同种原位肝移植术，历时１１小时３０分，手术获得成功。供肝热缺血时间３分１２秒，冷缺血时间９小时１０分，受者无肝期２小时１２分。术中对质地坚硬且体积巨大的肝脏仍采用左、右翻转法顺利处理第三肝门，肝静脉重建采用供肝肝上下腔静脉与受者下腔静脉行端侧吻合。术中在无体外静脉转流的情况下，血流动力学保持平稳，术后恢复良好。Piggyback allogenic orthotopic liver transplantation was performed successfully in one patient with primary liver carcinoma.The operation took 11 hours and 30 minutes.The donor liver was held in the state of heat ischemia for 3 minutes and 12 seconds,in the state of cold ischemia for 9 hours and 10 minutes,the recipient was held in the state of anhepatic for 2 hours and 12 minutes.The resection of the liver in the patient was initiated by the successful ligation and transection of the veins within the third porta hepatis,the reconstruction of the hepatic venous system was done with the end of the suprehepatic inferior vena cava anastomosized to the patient.Without extracorporeal venous bypass,the hemodynamics was held steadfast intraoperatively,the post-operative recovery was uneventful.With reference to the literatures,the operational techniques were reviewed.
