糖尿病肾病尿毒症患者腹膜平衡试验及其临床意义

摘　　要：目的：观察非胰岛素依赖性糖尿病肾病（２型ＤＮ）尿毒症腹膜透析（ＰＤ）患者中腹膜转运功能的变化及其意义。方法：应用腹膜平衡试验（ＰＥＴ）前瞻性观察１８例２型ＤＮ尿毒症患者在ＰＤ治疗３６个月内腹膜转运功能变化，患者分别于接受ＰＤ治疗１、６、２４、３６个月时测定ＰＥＴ一次，并同时检测１８例非糖尿病肾病尿毒症患者ＰＥＴ，进行对照分析。结果：在ＤＮ尿毒症患者中ＰＥＴ结果显示，ＰＤ治疗６个月内，５例呈高转运状态，１１例为高于平均转运，２例呈低于平均转运；ＰＤ治疗６个月时，１０例为低于平均转运，６例为高于平均转运，２例呈低转运；ＰＤ治疗２４个月时，大多数患者处于低于平均或低转运状态（Ｐ＜０．０５），而对照组患者在同时期ＰＥＴ检测结果无显著变化（Ｐ＞０．０５）。结论：２型ＤＮ尿毒症患者腹膜转运功能随ＰＤ时间延长而进行性降低，应参照ＰＥＴ结果调整ＰＤ的方案。OBJECTIVE The purpose of this investigation is to evaluate the changes of peritoneal equilibration test(PET)and its significance in peritoneal dialysis(PD)patients with non insulin dependent diabetes mellitus(NIDDM). METHODOLOGY This is a prospective controlled clinical study in which 18 newly peritoneal dialyzed NIDDM patients were included and followed up for 36 months.PET was performed at the end of the 1st,6th,24th and 36th month after the initiation of peritoneal dialysis.Eighteen peritoneal dialyzed non NIDDM patients were included as the control. RESULTS According to the results of PET at the end of the 1st month,5 patients in NIDDM group were categorized as high transport,11 as high average,2 as low average.While at the end of the 6th month,non in NIDDM group was grouped as high transport,6 as high average,10 as low average;and 2 as low transport.At the end of 24 months,most NIDDM patients were of low average or low transport( P <0 05).Meanwhile,PET of the control patients remained unchanged at the same duration( P >0 05). CONCLUSION The progressively decreased measurement of peritoneal equilibration test in PD patients with NIDDM revealed a decreased peritonium transportation in these patients.Dosage and type of peritoneal dialysis should be adjusted to the changes in patients PET.
