摘  要:目的:了解对亚胺培南耐药菌株对抗菌药物的敏感性及临床用药情况,为临床合理用药提供参考。方法:通过HIS系统调取2009年1—6月对亚胺培南耐药的83例患者的资料,记录患者基本情况及抗菌药物应用情况,经细菌学培养及药敏结果,分析对亚胺培南耐药的原因。结果:临床分离的耐亚胺培南菌株553株,其中铜绿假单胞菌18.63%(103/553)、鲍曼不动杆菌2.17%(12/553)、大肠埃希菌20.98%(116/553)、肺炎克雷伯菌13.21%(73/553)、鲍曼复合醋酸钙不动杆菌7.59%(42/553)、白色假丝酵母菌8.50%(47/553)、光滑念珠菌4.88%(27/553)、其他24.05%(133/553)等菌株。耐亚胺培南菌株对β内酰胺类抗菌药物具有高度耐药。结论:耐亚胺培南菌株大多存在多重耐-药性,临床应根据药敏试验结果选用对耐亚胺培南菌株敏感的抗菌药物。Objective: To study the sensitivity to antibiotics of imipenem-resistant strains, and clinical medication situation,and to rationalize the clinical medication. Methods: Through the HIS system from Jan. 2009 to Jun. on the 83 imipenem-resistant patients, we recorded patients basic information and the use of antimicrobial agents, statistics bacterial raise and sensitivity results, and analyzed the reasons for resistance to imipenem. Results: Five hundred and fifly-three(553) strains of imipenem-resistant were separated and the rates of resistance were Pseudomonas aenlginosa 18.63%(103/553), A.baumanii 2.17%(12/553), Escherichia coli 20.98%(116/553), Klebsiella pneumoniae 13.21%(73/553), A.calcoaceticus 7.59%(42/553), C.albicans 8.50%(47/553), Candida glabrata 4.88%0(27/553), and others 24.05%(133/553) respectively, lmipenem-resistant strains were highly resistant to β-lactam antibiotics. Conclusion: Most imipenem-resistant strains have multiple drug resistances and so people should select the antibiotics that sensitive to imipenem-resistant strains in the clinical medication.
