隐蔽部位肺癌的胸片细微征象分析

摘　　要：目的探讨隐蔽部位肺癌在胸片上的细微表现。方法回顾分析经CT及病理证实的隐蔽部位肺癌18例,均为DR摄片,分析其X线表现。结果胸片细微征象为:肺门形态轻微改变;隐蔽部位的小斑片,小结节,纤维条索,小空洞;半露或重叠的结节或肿块;血管周围模糊影;肺部相关间接征象。胸片检出率55.6%,诊断准确率33.3%,漏诊率38.9%,X线胸片阳性率94.4%。结论了解隐蔽部位肺癌的好居部位,认识胸片上的细微征象,常规摄DR正、侧位胸片,仔细有序读片,发现线索后追踪检查可提高隐蔽部位肺癌在胸片上的检出率和准确率。Objective To study the subtle signs of chest X-ray of lung cancer in hide place.Methods The features on the direct digital radiography（DR）were analyzed retrospectively in 18 patients with lung cancer in hide place proved by CT or pathology.Results The subtle signs of chest X-ray included slight change of hilar shape,small patches,small nodules,fiber stripe and small cavities in hide place,partly showed or overlapping nodule or mass,hazy shadow around blood vessels,related indirect signs of the lungs.Chest X-ray detection rate was 55.6%,accuracy rate of diagnosis was 33.3%,missed diagnosis rate was 38.9%,and positive rate was 94.4%.Conclusion The detection rate and accuracy of lung cancer in the hide place may be improved by learning the usual location of lung cancer in hide place,finding the subtle signs on the chest X-ray,reading the films seriously and orderly and searching for the clues,when posteroanterior and lateral chest X-ray were conventionally taken.
