静脉注射沐舒坦佐治胎粪吸入综合征的临床观察

摘　　要：目的观察静脉注射沐舒坦对胎粪吸入综合征(MAS)的疗效。方法对25例确诊新生儿胎粪吸入综合征且需要呼吸机治疗的患儿随机分为沐舒坦佐治组和常规治疗组,两组均给予机械通气和常规治疗,沐舒坦佐治组在常规治疗同时给予静脉注射沐舒坦(30 mg.kg-1.d-1),用药时间为5 d。观察用药后12、24、36、48、72、96、120 h分别记录各研究对象平均气道峰压(MAP)、氧合指数(PaO2/F iO2)的结果,并记录各研究对象的机械通气时间。结果沐舒坦佐治组与常规治疗组的MAP和PaO2/F iO2在用药后48、72、96、120 h差异有统计学意义(P<0.05)。两组的平均机械通气时间有统计学差异(P<0.05)。结论沐舒坦在用药48 h后能明显改善胎粪吸入患儿的肺通气和换气功能,降低机械通气时的平均气道压,缩短机械通气时间。Objective To observe the effect of Mucosolvin to cure meconium aspiration syndrome（MAS） by intravenous injection.Methods 25 newborns finally diagnosed MAS and requiring curing by breathing machine were divided into treat group and control group.The newborns of treat group were given Mucosolvin（30 mg/kg/d）.The medication time was 5 days.Then we observed MAP,PaO2/ FiO2 of every object at the each time point 12,24,36,48,72,96,120 h after given medication.The time of using breathing machine was also recorded in every group.Results There was significant difference between the two groups for MAP,PaO2/ FiO2 at 48,72,96,120 h after administration（P0.05 or 0.01）.The average time of mechanical ventilation was significant difference between the two groups（P0.05）.Conclusion Mucosolvin can improve ventilation air change function of MAS newborns,degrade mean airway pressure,decurtate the time of mechanical ventilation.
