自制吸氧头罩早期高压氧治疗对重型颅脑损伤应激性溃疡发生率的影响

摘　　要：目的:探讨早期吸氧头罩法高压氧治疗重型颅脑损伤并发应激性溃疡出血的作用。方法:分析162例重型颅脑损伤患者(GCS评分3～8分)临床资料,随机将患者分为对照组和观察组,对照组给予常规治疗,用甲氰咪胍静脉滴注预防应激性溃疡出血,观察组除按对照组治疗外,早期应用吸氧头罩进行高压氧治疗,观察应激性溃疡的发生及预后情况。结果:观察组与对照组比较,患者发生应激性溃疡出血的发生率明显下降,两组相比,差异有统计学意义(P<0.05)。结论:颅脑损伤愈重,GCS评分愈低,应激性溃疡出血的发生率愈高,应用吸氧头罩法早期高压氧综合治疗,可降低应激性溃疡出血的发生率及高病死率。Objective：To explore the prevent effect of hyperbaric oxygenation therapy in incidence of stress ulcer induced to severely brain injured patients with self-designed head oxygen cover.Methods：162 cases of severely brain injured patients（Glasgow coma scale from 3 to 8） were divided into the observe group and the control group at random.The control group＇s patients received cimetidine to prevent or to treat the stress ulcer.While,to the observe group＇s,received early hyperbaric oxygenation therapy with self-designed head oxygen cover.Results：The incidence of stress ulcer was lower in the observe group compared with the control group（P〈0.05）.Conclusion：The incidence of stress ulcer may be higher if brain injured is severer or glasgow coma scale is lower.It may be help to reduce the incidence of stress ulcer and the high mor tality rate due to the stress ulcer with early hyperbaric oxygenation therapy to severely brain injured patients.
