无堵塞引流管强负压吸引治疗消化道瘘

摘　　要：目的探讨和评价应用无堵塞引流管,封闭皮肤瘘口,强负压吸引治疗消化道瘘的效果和临床价值。方法2006年10月至2009年9月采用自行研制的无堵塞引流管和规范治疗方法治疗消化道瘘9例,观察消化道瘘治疗过程中每日引流量的变化、治愈时间、是否有引流管堵塞及并发症发生等情况。结果本组9例消化道瘘患者均治愈,平均愈合时间10.7 d。治疗过程中,未发现引流管堵塞情况,未见腹腔内出血、腹腔感染、肠梗阻及败血症等并发症。结论采用无堵塞引流管不仅能够保证瘘管充分有效地引流,同时,强负压可以达到闭合软组织内死腔、促进深部组织粘结,进而达到吸引瘘口周围软组织压迫堵塞,直接闭合瘘口,恢复消化道完整性的治疗效果,是一种消化道瘘早期引流性治疗的理想方法。Objective To explore and evaluate the effect and clinical value of the no obstructed drainage tube,seal-up of the fistula of skin,high negative pressure drainage in the treatment of digestive tract fistula.Methods From oct.2006 to sep.2009,9 patients with digestive tract fistula were treated with the no obstructed drainage;our department designed all products.The amount of the effusion,the healing time,the drainage tube blockage and the complications were observed.Results Total 9 patients were healed,the average recovery time was 10.7days.No case of the drainage tube was obstructed;No complications of bleeding,abdomen infection,ileus and sepsis were found.Conclusion The no obstructed drainage can quarantine the sufficient and effective drainage,and high negative pressure can directly close the fistula and recover the integrity of digestive tract.It is a perfect drainage method for treating digestive tract fistula at early stage.
