摘  要:目的:考察普罗帕酮(PPF)在心律失常患者人群中存在的代谢差异.方法:用高效液相色谱法测定心律失常患者服药后体内的PPF和5-羟普罗帕酮(5-OHP)的血药浓度.结果:在各剂量组中5-0HP和PPF的稳态谷值均具有较大的个体差异.随着剂量的增加,PPF血浓呈线性增加,而5-OHP的浓度变化较小.结论;约有4.6%患者的5-OHP血浓超过PPF.低剂量组中5-OHP的体内积聚较为明显.本研究中弱代议的发生率约为3.25%,其稳态谷值浓度相对较高,在大剂量用药时应注意毒性反应.AIM:To study propafenone metabolism in arrhythmic patients. METHODS: Serum concentration of propafenone and 5-hydroxypropafenone (5-OHP) were determined in arrhythmic patients by HPLC. RESULTS:The results showed that there were diffrernces a-mong patients in 5-OHP and propafenone steady state though concentration. With the dose of propafenone increasing, the serum concentration of propafenone increased linearly. There were little changes in the concentration of 5-OHP. CONCLUSION: The concentration of 5-OHP is greater than propafenone in 4.6% of 615 patients received test. Accumulation of 5-OHP is obvious in the lower dosage group. Poor metabolism is found in 3. 7% of patients. Be-cause of the higher steady state trough concentration of propafenone, the toxic side effects should be carefully prevented when the larger doses of propafenone are used.
