超声引导下外周型肺病变及胸腔肿瘤穿刺活检

摘　　要：目的评价超声引导下穿刺活检在外周型肺病变及胸腔肿瘤中的诊断作用和安全性。方法回顾性分析多种肺及胸腔病变共30例,在超声引导下用Bard弹射式自动活检枪和18G活检针、配合使用ALOKA9133小凸阵探头进行经皮穿刺活检。标本送组织病理检查,结果与手术病理及临床随访的最后诊断进行比较。结果30例患者,穿刺活检成功率为100%,组织取材满意率93.5%,诊断准确性为100%,穿刺后并发症共2例、咯血2例。结论超声引导下对外周型肺病变及胸腔肿瘤穿刺活检具有定位准确、操作简便、安全性好、并发症少及确诊率高等优点,使多数病变获得了明确诊断,有助于临床进一步治疗。Objecitve To appraise ultrasound（US） guided tissue biopsy＇s diagnosis function and security in patients with concrete lesion around lung or in pleura. Methods Review and analysis 30 various lungs and thorax pathological changes, US guided tissue biopsy with Bard automatic pop - up rifle and 18G biopsy needle, 9133 light convex array search unit. Sending specimen to do histopathological examination. Comparing the result with operation pathology and clinical follow - up examination. Results Those 30 patients, it is 100% that among them it is alive to puncture and examine success rate, 93.5 % of satisfactory -rate that the organization draws materials, diagnose accuracy is 100%, complication amount to 2 after the puncture, 2 of emptysis. Conclusion US guided tissue biopsy changes to the concrete lesion around lung or in pleura has a positioning accuracy, easy operation, safety is good, less complications and high diagnostic rate of benefits, and it is important for patients with concrete lesion around lung or in pleura. It can facilitate further treating in clinic.
