摘 要:目的为了确定磁刺激腰骶部神经根的兴奋点位置,采用两种方法测定运动诱发电位(MEPs)的潜伏期,并用磁刺激MEPs与磁刺激诱发电位的F波相结合测定运动神经根传导时间(MRCT)。方法在50例正常受试者和40例腰椎间盘突出症患者,从胫前肌(TA)和比目鱼肌(SOL),测定MRCT和运动神经传导时间(MNCT)。5例尸解测出L_5和S_1神经根长度。结果磁刺激腰骶部神经根的兴奋点在椎间孔的近端;患者组与正常组的MNCT相比无显著性差异(P>0.05),但MRCT两组比较有显著性差异(P<0.01),异常率为85%。结论磁刺激腰骶部为椎间孔处神经根刺激,这与电刺激相同。磁刺激MEPs对腰椎间盘突出症有很大的诊断价值。Objective Two methods were used to detect the latency of motor evoked potentials(MEPs) in order to locate the exciting point of nerve root by magnetic stimulation in lumbosacral region. Motor root conduction time(MRCT) was calculated by combining the magnetic stimulation elicited MEPs in lumbosacral region with the same stimulation elicited F wave in the popliteal fossa. Methods Fifty normal subjects and forty patients suffering from disc herniation were studied, the length of L_5 and S_1 nerve root was measured in five corpses of male adults. MRCT and motor nerve conduction time(MNCT) were recorded from tibialis anterior(TA) and the soleus(SOL). Results The exciting point of nerve root by magnetic stimulation was at the closed end of intervertebral foramen; No difference was seen in MNCT in all patients but MRCT was markedly prolonged, in 85% of patients. Conclusion Stimulating the lumbosacral region by magnetic coid is same as the eletrie stimulation on intervertebral foramen nerve root. Magnetic stimulation of MEPs is a useful technique for non-invasive diagnosis of lumbar disc herniation.
