母婴同室新生儿医院感染预防与控制方法研究

摘　　要：目的探讨母婴同室新生儿医院感染预防与控制的有效方法。方法在6年时间里,医院感染管理科拟定预防和控制医院感染制度并检查相关制度落实情况,加强对医院感染病例、消毒灭菌效果及母婴同室的环境卫生学监测,同时与相关科室密切配合,全员参与,共同完成新生儿医院感染预防与控制工作。结果在新生儿医院感染病例中,皮肤感染、新生儿脐炎、新生儿脓疱疹明显下降,由2003年的2.07%降至2008年的0.17%,经线性趋势χ2检验统计处理差异有统计学意义(P<0.01);新生儿医院感染率由2003年的5.7%降至2008年的0.86%,漏报率逐年下降,经线性趋势χ2检验统计学处理差异有统计学意义(P<0.01);母婴同室有关部位的环境卫生学监测均符合医院要求;母婴同室新生儿无医院感染流行及暴发流行。结论医院感染管理科制定并督促落实相关制度、加强监测、并与有关科室密切配合和全员参与,是预防与控制医院感染的有效方法。OBJECTIVE To explore the effective way to prevent and control neonatal nosocomial infection in direct rooming-in wards. METHODS From the past 6 years,nosocomial infection managers have studied out measures to prevent and control hospital infection,supervised the implementation of relevant systems,and strengthened sanitary monitor of nosocomial infection cases,sterilization effect and direct rooming-in wards. Simultaneously,in close cooperation with related departments,all staff participated in completing work of prevention and control of neonatal nosocomial infection. RESULTS In neonatal nosocomial infection cases,the incidence of skin infection （neonatal omphalitis and neonatal pustular eruption）declined significantly （P〈0.01,by linear chi-square test）. The rate of neonatal nosocomial infection and miss report reduced year by year （P〈0.01,by linear chi-square test）. The sanitary monitoring results of departments related to direct rooming -in wards conformed to hospital requirement. There was no epidemic or outbreak of neonatal nosocomial infection in direct rooming-in wards. CONCLUSIONS It is an effective way to prevent and control nosocomial infection that nosocomial infection management department formulates relevant systems and supervises the implementation ,strengthens monitoring,and coordinates closely with relevant departments and all staff participate in this work.
