摘  要:目的:探讨外加低压稳恒直流电场对兔心肌梗死(MI)后心肌基质金属蛋白酶-2(MMP-2)表达的影响及其对左室重构的作用。方法:42只日本大耳白兔,随机分为假手术组(SH组)、MI组和电治疗组(EFs组)。建立急性MI或假手术模型后,在回旋支两侧心外膜上缝合一对铂金电极,EFs组直流电刺激(4.0V/cm,30min/d)。实验终点测定兔左、右室重量和血流动力学指标,Western-blot以及RT-PCR检测梗死边缘区心肌组织MMP-2表达。结果:MI组左室心肌肥厚指数升高,同时左室收缩功能较SH组下降(P<0.05),EFs4周组左室心肌肥厚指数降低而左室收缩功能提高。MI组MMP-2表达随时间而增高,EFs4周组基质金属蛋白酶-2的表达水平较同时相点MI组显著下降(P<0.05)。结论:外加低压稳恒直流电场治疗可通过降低梗死边缘区心肌组织MMP-2的表达而改善MI后左室重构和心功能。Objective:To observe the effects of applied steady low-voltage direct current (DC) electric fields (EFs) on the expression of matrix metalloproteinase-2 (MMP-2) and the left ventricular remolding in a rabbit model of myocardial infarction. Method:Forty-two rabbits were randomly divided into sham operation group (SH group),control group (MI group) and treatment group (EFs group). After the corresponding model was established,DC EFs (4.0 V/cm,30 min/day) was applied to the EFs group from the 2nd postoperative day to the end of the experiment. The ratio of left or right ventricular weight/body weight (LVW/BW or RVW/BW) and the hemodynamics index as well as the expression of MMP-2 of the myocardium in the infarct marginal zone were detected with Western-blot and RT-PCR methods.Result:The ratio of LVW/BW and the expression of MMP-2 were all increased while the systolic function of left ventricle (LV) was decreased in the MI group compared with the SH group(P〈0.05).After the application of the DC EFs,the LVW/BW and the LV systolic function of MI rabbits were improved at the fourth postoperative week,and at the same time,the expression of MMP-2 of the myocardium in the marginal zone was decreased. Conclusion:DC EFs can improve the left ventricular remodeling and cardiac function by inhibiting the expression of MMP-2 in the marginal zone myocardium after myocardial infarction.
