摘 要:目的研究临床标本中分离的粪肠球菌耐药特性及其ermB基因与转座子Tn1545和Tn917的相互关系。方法采用K-B纸片法对临床分离的86株粪肠球菌进行抗菌药物敏感性试验,用PCR检测粪肠球菌ermB、Tn1545和Tn917基因。结果86株临床分离的粪肠球菌对红霉素的耐药率为74.4%;耐红霉素粪肠球菌中,ermB基因的携带率为95.3%,Tn1545的携带率为81.3%,Tn917的携带率为26.6%;其中,26.2%的ermB基因同时存在于Tn1545和Tn917中,55.4%的ermB基因单纯存在于Tn1545中,1.5%的ermB基因单纯存在于Tn917中。结论粪肠球菌耐大环内酯类抗菌药物的主要耐药基因为ermB基因,ermB基因主要是通过Tn1545来携带其对大环内酯类抗菌药物的耐药性,并且Tnl545可能是引起粪肠球菌多药耐药的重要机制之一。OBJECTIVE To determine the drug-resistance rate of 86 Enterococcus faecalis strains isolated from patients of our hospital,and to characterize the retation ship between ermB gene and Tn1545 and Tn917 transporison in E.faecalis.METHODS The antimicrobial susceptibility to antibiotics of E.faecalis isolates was determined,and PCR was used to test ermB gene and Tn1545 and Tn917 of E.faecalis isolates.RESULTS Among all of the 86 E.faecalis isolates,the resistance rate to erythromycin was 74.4%.The ermB gene carriage rate in erythromycin-resistant E.faecalis was 95.3%.The Tn1545 and Tn917 gene carriage rate was 81.3%,and 26.6%,respectively.The ermB gene existed in Tn1545 and Tn917 only in Tn1545 was 55.4% only in Tn917 was 26.2%,55.4% and 1.5%,respectively.CONCLUSIONS Resistance to macrolides in E.faecalis isolates is generally mediated via ermB gene,which is mainly transported by Tn1545.There is a strong relationship of the multidrug resistance with Tn1545-related elements.
