摘  要:目的:通过检测健康人24小时固定时间点的血小板聚集率,观察其有无节律变化,为临床使用抗血小板药物时间提供合理依据。方法:30例健康年轻人入选。于1天24小时内2∶00、6∶00、10∶00、16∶00和20∶00采取外周静脉血,抽血后1小时内用全血阻抗法检测血小板聚集率,诱导剂分别为胶原(2μg/ml)、肾上腺素(10μg/ml)、花生四烯酸(0.5mM)及二磷酸腺苷(10μM及20μM)。结果:健康人血小板聚集率在24小时内,肾上腺素诱导下6∶00最高,与2∶00,10∶00,16∶00和20∶00时比较,差异有统计学意义(P均<0.05);其它时间点之间比较差异均无统计学意义。胶原、花生四烯酸及二磷酸腺苷诱导下血小板聚集率各时间点之间比较差异均无统计学意义。结论:肾上腺素诱导的健康人群血小板聚集率存在24小时节律变化,早晨6∶00起床后达到高峰,此后逐渐降低,夜间睡眠中达到低谷。胶原、花生四烯酸及二磷酸腺苷诱导的血小板聚集率无明显24小时节律变化。Objective : To detect platelet aggregation of circadian variation in fixed time point of the same day in healthy subjects in order to rationally use anti-platelet medication in clinical practice. Methods :A total of 30 healthy volunteers were enrolled. Blood samples were obtained at 2: 00,6: 00,10: 00,16:00 ,and 20:00 o' clock in the same day. Platelet aggregation was assessed within 1 hour after blood was taken. Platelet aggregation was induced by collagen (2 μg/ml),epinephrine (10 μg/ml),arachidonic acid (0.5 mM) and ADP (10 μM and 20 μM respectively). Results:In healthy subjects, compared with other time points, epinephrine induced platelet aggregation was the highest at 6:00 o' clock (P = 0.05, respectivety). While collagen, arachidonic acid and ADP induced platelet aggregation showed no significant changes at different time point. Conclusion:Platelet aggregation presented a specific circadian rhythm in healthy subjects. Epinephrine induced platelet aggregation showed the highest value at 6:00 AM,while collagen, ADP and arachidonic acid induced platelet aggregation had no significant circadian variation.
