胆囊结石与胆囊息肉患者术后远期生存质量的研究

摘　　要：目的了解不同术式对胆囊结石及胆囊息肉患者生存质量的影响。方法采用消化病生存质量指数（ Gastrointestinal Quality of Life Index, GLQI）测定289例采用不同术式治疗的胆囊结石及胆囊息肉患者术后1年以上的生存质量值。结果289例患者的GLQI指数平均分为114．0分，明显低于正常人群124．5分，（P〈0．01）。230例胆囊结石患者术后GLQI指数平均分为113．8分，36例胆囊息肉患者术后GLQI指数平均分为117．2分，23例胆囊结石合并胆囊息肉患者术后GLQI指数平均分为110．4分，但三者之间的比较差异无显著性（P〉0．05）。保胆术后患者GLQI指数为117．8分，胆囊切除性手术患者术后GLQI指数为109．6分，两者之间差异具有显著性（P〈0．01）；开腹胆囊切除术患者术后GLQI指数为109．6分，腹腔镜胆囊切除术患者术后GLQI指数为109．6分，两者之间比较差异无显著性（P〉0．05）。结论保胆手术术后患者生存质量明显高于胆囊切除性手术患者，切除胆囊能使患者的生存质量明显下降。Objective To evaluate the influence of different operative way on the quality of life of post- operative patients who suffered from cholelithiasis and gallbladder polyps. Methods Values in the quality of life of 289 post-operative patients who suffered from cholelithiasis and gallbladder polyps were tested by using gastrointestinal quality of life index. Results The mean GLQI score of 289 patients is 114. 0, and much less than that of normal cases whose GLQI score is 124. 5 （P 〈 0. 01 ）. The mean GLQI score of 230 patients with cholelithiasis is 113.8, the mean GLQI score of 36 patients with gallbladder polyps is 117.2, and the mean GLQI score of 23 patients with cholelithiasis and gallbladder polyps is 110. 4. The mean GLQI score of patients who underwent gallbladder-preserving operation is 117. 8, and the mean GLQI score of patients who underwent ehole- cystectomy is 109. 6, and there were significant difference （P 〈 0. 01 ）. The mean GLQI score of patients who underwent open eholecystectomy is 109. 6, and the mean GLQI score of patients who underwent laparascopie chole- cysteetomy is 109.6, and there were not significant difference （ P 〉 0.05 ）. Conclusion The quality of life of patients with cholelithiasis who underwent gallbladder-preserving operation is better than that of patients who underwent eholecystectomy. To excise gallbladder decreases the quality of life of patients.
