摘 要:目的探讨有创机械通气治疗连枷胸合并急性肺挫伤的疗效。方法回顾2006年1月至2009年7月连枷胸合并肺挫伤的患者18例,行呼吸机内固定治疗后分析血气分析等各种参数的变化。结果行呼吸机内固定治疗后动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO2)、氧合指数(PaO2/FiO2)和脉搏血氧饱和度(SpO2)均较前有明显改善(P<0.01)。本组治愈14例,治愈率77.8%。结论有创机械通气不仅能固定连枷胸,而且可有效地治疗肺挫伤,但应注重控制机械通气时间,防治相关并发症。Objective To study the effect of invasive mechanical ventilation on the treatment of flail chest complicated with acute pulmonary contusion. Methods 18 cases of flail chest complicated with acute contusion from January 2006 to July 2009 were retrospectively analyzed; the Changes of all kinds of parameters during the respirator internal fixation treatment were analyzed. Results PaO2,PaCO2,PaO2 /FiO2 and SpO2 were obviously improved after the treatment,and there was statistical significance (P〈0. 01). 14 cases were cured,the cure rate was 77. 8% ,Conclusion Invasive mechanical ventilation can not only fix flail chest,but also effectively treat pulmonary contusion,but need to control the duration of mechanical ventilation and prevent and cure the relative complications.
