曲美他嗪治疗充血性心力衰竭的临床观察

摘　　要：目的探讨曲美他嗪对充血性心力衰竭患者的影响。方法 30例缺血性心肌病和扩张性心肌病在常规治疗(洋地黄、利尿剂、血管紧张素转换酶抑制剂、硝酸酯类)基础上,口服曲美他嗪20mg,3次/d,6周后观察临床疗效。超声心动图测定治疗前后左室舒张末期内径(LVEDD)、左室射血分数(LVEF)及心功能的变化。结果治疗后LVEDD明显下降[(68.3±7.2)mm vs(53.2±5.4)mm,P<0.01],LVEF增加[(0.36±0.02)%vs(0.49±0.04)%,P<0.05],心功能改善1～2级。无发现与药物相关的不良反应。结论曲美他嗪能改善缺血性心肌病和扩张性心肌病的心功能。Objective To explore the effects of trimetazidine on congestive heart failure in patients with ischemic cardiomyopathy and dilated cardiomyopathy. Methods Thirty cardiomyopathy patients （ 16 with ischemic cardiomyopathy and 14 with dilated cardiomyopathy）took trimetazidine 20rag after each of the three meals, besides routine therapy. Clinical effects were evaluated after 6 weeks of treatment. Left veutricular end diastolic diameter （LVEDD）,left ventricular ejection fraction （LVEF） of echocardiography and cardiac function （NYHA）were measured before and after the treatment of trimetazidine. Results After 6 weeks of therapy, LVEDD decreased significaently [ （68. 3 ±7.2）mm vs （53.2 ±5.4）mm,P 〈 0. 01 ], LVEF increased[ （0. 36± 0.02）% vs （0.49 ±0.04）% ,P 〈 0. 05] ,cardiac function（NYHA） improved 1 - 2 grade. There were no adverse reactions related to the drug. Conclusion Trimetazidine can improve the heart failure in patients with ischemic cardiomyopathy and dilated cardiomyopathy.
