两种方法治疗糖尿病合并慢性心力衰竭的评价

摘　　要：目的:探讨两种方法治疗糖尿病合并慢性心力衰竭的效果。方法:90例2型糖尿病合并慢性心力衰竭患者随机分为卡维地洛治疗组(A组)45例和常规治疗组(B组)45例。B组采用常规治疗,A组在B组治疗的基础上加服卡维地洛25mg,2次/d,比较治疗后两组患者的心率、血压、心脏功能、心脏事件及药物耐受性。结果:A组治疗后LVESVI、LVEDVI、LVEF较治疗前有显著性差异(t=2.231,t=2.311,t=2.351,均P<0.05);A组治疗后LVESVI较B组有显著性差异(t=2.312,P<0.05);两组6min步行距离治疗后较治疗前有显著性差异(t=2.345,P<0.05);A组6min步行距离治疗后较B组比较有显著性差异(t=2.312,P<0.05);A组和B组低血糖事件发生率(10.2%和12.5%)比较,无显著性差异(P>0.05)。结论:卡维地洛治疗糖尿病合并慢性心力衰竭的安全性和有效性明显优于常规治疗。Objective： To investigate the curative effect of diabetic mellitus with chronic heart failure by two treatment methods. Methods： 90 patients of diabetes mellitus with chronic heart failure were randomly divided into Carvedilol treatment （Group A） 45 cases and Conventional treatment （Group B） 45 cases. The Group B with conventional therapy, Group A in the control group based on the added Carvedilol 25 mg, 2 times/d, the heart rate, blood pressure, heart function, cardiac events and drug tolerance in two groups after treatment were compared. Results： The indicators of LVESVI, LVEDVI, LVEF after treatment than before treatment were significantly different in Group A （t=2.231, t=2.311, t=2.351, all P〈0.05）; the indicators of LVESVI in Group A compared to the Group B after treatment, there was significant difference （t=2.312, P〈0.05）; the indicators of 6 min walk in two groups after treatment compared to before treatment, there was significant difference （t=2.345, P〈0.05）; the indicators of 6 min walk in Group A compared to the Group B after treatment, there was significant difference （t=2.312, P〈0.05）; the incidence of hypoglycemia in Group A and Group B （10.2%, 12.5%） were compared, there was no significant difference （P〈0.05）. Conclusion： The safety and effectiveness of Carvedilol treatment were superior than Conventional treatment in diabetes mellitus with chronic heart failure.
