汶川地震伤员在综合医院救治时心理状况的调查分析

摘　　要：目的:探讨汶川地震事件后转送到综合医院救治的伤员心理障碍的发生情况、躯体形式障碍症状特点及其转归。方法:根据《中国精神障碍分类与诊断标准(第三版)》(CCMD-3)的急性应激障碍(ASD)和抑郁焦虑情感障碍诊断标准,对接受治疗的50例伤员进行临床诊断,采用汉密尔顿抑郁量表、汉密尔顿焦虑量表、匹兹堡睡眠指数、社会支持量表、医学应对量表、日常生活活动能力巴氏指数、症状记录表等进行评定。并于1年后电话随访。结果:50例伤员中有16例符合急性应激障碍(ASD)、2例符合抑郁情感障碍、2例符合焦虑情感障碍诊断标准,1例出现躯体形式疼痛障碍。1年后电话随访患者,创伤后应激障碍(PTSD)部分症状仍然存在。结论:汶川地震事件后的伤员,虽得到了积极救援治疗和社会各界关怀、支持,但仍然出现了急性应激障碍(ASD)和抑郁焦虑情感障碍,并有发生PTSD的可能,说明积极关注灾后伤员心理健康并对心理障碍积极干预治疗是非常必要的。Objective：To investigate the occurrence condition of the mental disorder, symptomatic feature of somatoform disorder and outcome of the rescued wounded personnel transferred to a General Hospital of Shanxi Province after Wenchuan earthquake, nethods：A total of 50 rescued wounded personnel were carried out the clinical diagnosis and assessed by Hamilton Depression Scale （HAMD）, Hamilton anxiety scale （HAMA）, Pittsburgh Sleep Quality Index （PSQI） ,Social Support Scale, Medical Coping Modes Questionnaire（MCMQ） ,Activities of Daily Living Scale （ADL） and Symptoms Recorded Scale according to the disorder of the Criteria of Chinese Classification diagnostic criteria on acute stress disorder and depression and anxiety and Diagnosis of Mental Diseases-3rd Edition and then interviewed by telephone after one year. Results：Sixteen cases accorded with the diagnostic criteria of acute stress disorder（ASD） ,2 cases accorded with the diagnostic criteria of depression disorder,2 cases accorded with the diagnostic criteria of anxiety disorder and one case was somatoform pain disorder among the 50 wounded personnel. The rescued wounded personnel were interviewed by telephone 1 year later and found the part symptoms of post-traumatic stress disorder（PTSD） still existed. Conclusion： Though the wounded personnel in Wenchuan earthquake accepted the treatment of active rescue and more supports and concerns of all society, they still have some acute stress disorder, depression and anxiety disorder and the post-traumatic stress disorder may occur. So it is necessary to pay more attention to their mental health and to perform the active intervention treatment for the mental disorder.
