血清胱抑素C在老年原发性高血压患者早期肾功能损害监测的临床意义研究

摘　　要：目的了解血清胱抑素C在老年原发性高血压患者早期肾功能损害监测的临床意义。方法检测72例老年高血压病患者和40例无高血压、糖尿病及肾病的健康老年对照组BUN、Scr、Ucr、UMA和ScysC值,并用Scr、Ucr、体表面积和24h尿量计算出CCr,将结果作统计学处理。结果患者组的BUN、Scr、UMA、ScysC和CCr均值均显著高于对照组(P均<0.01),BUN、Scr和UMA异常检出率均<40%,明显低于ScysC和CCr(P<0.05),其中以ScysC异常检出率最高,达58.33%(42例),在Scr和UMA正常者中尚有13例(29.55%)ScysC及1l例(25.00%)CCr异常。结论ScysC和CCr值检测对老年原发性高血压患者早期肾功能损害的预测有指导意义,但以ScysC临床适用性较好。Objective To estimate the clinical significance of serum cystatin C to the early renal function impairment of the elderly patients which were the primary hypertension. Methods From 2008 Dce. to 2009 Jun, total of 72 elderly patients which was hypertension was patient group and 40 cases which were no hypertension and no diabetes was control group. The blood pressure （BP）, blood urea nitrogen （BUN）, serum creatinine （Scr）, urine microalbumin （UMA）, serum cystatin C（Scycs C） and body height and weight were detected and creatinine clearance（CCr） was calculated according to Scr, uric creatinine（Ucr）,24 h uric volume and body height and weight . All results were handled by statistics. ResultsThe averages of BUN, Scr, UMA, Scys C and CCr in the patient group were remarkably higher than that of those in the control group（P〈0.01）, and the abnormal rates of BUN, Scr and UMA were remarkably lower than that of Scys C and CCr（P〈0.05）. The abnormal rate of Scys C was 58.33% and that of CCr was 54.17%. In the patients with normal Scr, the abnormal rates of Scys C and CCr were 29.55% and 25.00% within the patient group. Conclusion Scys C and CCr possess remarkable value on the early diagnosis of the renal function injury in the elderly patient with the primary hypertension and Scys C is the most remarkable. The clinically practicable effect of Scys C is better.
