应用甲基强的松龙对颈椎疾患减压术后神经功能的影响

摘  要:目的:探讨在颈椎疾患减压术中应用甲基强的松龙(MP)对其神经功能恢复的影响。方法:选取伴有脊髓受压的颈椎疾患患者42例,并将其随机分为2组,A组:MP治疗组,22例,手术减压前30min静脉快速滴注MP1000mg;术后1h开始静脉应用MP(80mg/d),每天2次,连续使用3d。B组:对照组,20例,施行同样减压术未使用MP。以脊髓功能JOA评分和相关并发症发生率作为标准进行前后对比研究。所有病例均随访至少6个月,并定期进行神经功能评估。结果:2组患者术前JOA评分分别为(5.7±3.1)分和(6.1±2.4)分,2组间无统计学意义(P>0.05)。术后2组JOA评分分别为(10.9±3.3)分和(8.7±2.3)分,2组相比有统计学意义(P<0.05)。2组均未出现消化道溃疡、感染、心血管系统衰竭等并发症。结论:应用甲基强的松龙对伴有脊髓受压的颈椎疾患减压术后神经功能有明显改善作用。Objective:To evaluate the effect of methy1prednisolone(MP)on neural function recovery after decompression procedure of the cervical disorders.Method:42 cases of cervical disorders with spinal cord compression were selected and randomly divided into two groups.22 cases in experimental group were treated with MP before and after decompression procedure.1000mg MP was administered within 30 min before operation and 80mg MP was used in the first,the second and the third day after operation for two times a day.The other 20 cases were not administered with MP as a control.The two groups were compared by analyzing the functional evaluation of spinal cord(established by Japan orthopedic association,JOA)and incidence rate of complications related to MP.Each case was followed at least for 6 months and neural function was assessed.Result:JOA scores of the two groups before surgery were 5.7±3.1 and 6.1±2.4 respectively and there was no remarkable significance(P0.05).However,the difference between the JOA scores of two groups after surgery were significant(10.9±3.3 vs 8.7±2.3,P0.05).Digestive ulcer,infection and cardiac failure related to MP did not appear in two groups.Conclusion:Administration of methylprednisolone before and after decompression procedure can significantly improve spinal cord function of patients with cervical disorders.
