摘  要:目的:探讨失血性休克大鼠肝脏组织组织因子(TF)、血栓调节蛋白(TM)、组织型纤溶酶原激活剂(t-PA)和纤溶酶原激活剂抑制物-1(PAI-1)的表达情况以及参附注射液的干预作用。方法:成年健康SD大鼠分成4组:对照组、休克组、林格液组、参附液组。对照组常规饲养,其余3组大鼠复制失血性休克模型。休克组不予治疗,林格液组用3倍失血量的林格液治疗,参附组用含参附注射液(10 mL/kg)和林格液组成的3倍失血量的液体治疗。治疗后2 h取肝脏组织,-70℃保存,用逆转录多聚酶链反应(RT-PCR)法检测肝脏组织中t-PA,PAI-1和TF,TM mRNA的表达。结果:①休克组肝脏组织TF,TM,t-PA,PAI-1 mRNA表达与对照组相比明显升高(P<0.05)。②林格液组肝脏组织TF,TM,t-PA,PAI-1 mRNA表达与对照组及休克组比较明显升高(P<0.05)。③参附液组肝脏组织TF,TM,t-PA,PAI-1 mRNA与对照组相比较,差异无显著性,与休克组及林格液组比较表达有降低,差异有显著性(P<0.05)。结论:休克状态下肝脏组织及内皮细胞损伤持续存在,参附注射液可使凝血活性下降,同时又可防止纤溶过度亢进。Objective:To explore the expression of tissue factor(TF),thrombomodulin(TM),tissue plasminogen activator(t-PA),plasminogen activator inhibitor-1(PAI-1) in rat liver after hemorrhagic shock,and the effect of Shenfu injection on them.Methods:Sprague-Dawley rats were divided into four groups:control group,hemorrhagic shock(HS group),resuscitation with Ringer’s solution(RS group),and resuscitation with Shenfu injection(SF group).The rats in control group were treated with regular feeding;the rest groups were subjected to hemorrhagic shock.Then,HS group was left without treatment,RS group was treated with Ringer’s solution with three times of volume of lost blood,SF group was treated with mixture of Shenfu injection(10 mL/kg) and Ringer’ s solution with three times of volume of lost blood.The livers were collected after 2 hours of the treatment and stored at-70 ℃.The mRNA levels of tissue plasminogen activator antigen(t-PA:Ag),plasminogen activator inhibitor-1 antigen(PAI-1:Ag),thrombomodulin(TM)and tissue factor(TF) were measured by reverse transcription polymerase chain reaction(RT-PCR).Results:Compared to the control group,mRNA levels of TF,TM,t-PA,PAI-1mRNA of HS group in liver were significantly increased(P 0.05);The mRNA levels of TF,TM,t-PA,PAI-1 in RS group increased significantly compared to those of control group(P 0.05) and HS group(P 0.05).The mRNA levels of TF,TM,t-PA,PAI-1 in SF group were significantly lower than those of RS group and the HS group(P 0.05),but there was not statistical difference compared to the control group.Conclusion:When hemorrhagic shock happens,the injury of liver and endothelial are persistently exist.Shenfu injection can prevent the formation of thrombus,improve microcirculation and less bleeding.
