年龄对ICU机械通气患者记忆和舒适度的影响

摘　　要：目的探讨年龄对ICU机械通气患者记忆程度和舒适度的影响。方法ICU机械通气超过24 h的患者,转出ICU 3-7 d内行回顾调查。按年龄分为〈30岁（A组,n=22）、30-60岁（B组,n=45）和〉60岁（C组,n=46）三组,分析年龄对机械通气和气道吸引记忆程度和舒适度的影响。结果113例患者纳入,C组患者APACHEⅡ评分（19.00±9.15）分明显高于A、B组[（8.38±6.37）分（、13.38±5.93）分]（P〈0.05）。A、B组患者ICU住院时间[（3.89±4.99）d、（3.38±3.63）d],明显短于C组[（7.46±14.47）d]（P〈0.05）。C组患者记得机械通气和气道内吸引的比例最低,仅为47.8%和21.7%,明显低于A、B组（P〈0.01）。B组患者机械通气痛苦指数为（2.58±3.11）,明显低于A组和C组[（6.80±2.42）和（7.43±2.28）]（P〈0.01）。结论随着年龄的增加,患者对机械通气和气道吸引的记忆明显减少。Objective To assess the effect of age on recall and discomfort in ICU patients under mechanical ventilation（MV）.Methods Patients in ICU expected to require MV for greater than 24 h received a sedative regimen to achieve a target Ramsay sedation score set by physician.The patients were interviewed in 3 to 7 days after discharged from ICU.The discomfort recall about MV and suctioning was evaluated by visual analog scale（VAS）.Results One hundred and thirteen patients with MV were enrolled in the study.APACHE Ⅱ score was（19.00±9.15） in group C,which was significantly higher than（8.38±6.37） in group A or（13.38±5.93） in group B（P〈0.05）.The days of ICU stay were（3.89±4.99） days in group A and（3.38±3.63） days in group B,which was significantly shorter than（7.46±14.47） days in group C（P〈0.05）.The percentages of recall about MV and suctioning were 47.8% and 21.7% in group C,which were the lowest compared to group A and group B（P0.01）.The numeric rating scale（NRS） of suffering from MV was（2.58±3.11） in group B,which was significantly lower than（6.80±2.42） in group C or（7.43±2.28）in group A（P〈0.01）.Conclusion As the age increases,the recall about MV and suction in ICU decreases.
