武汉市常青花园原发性高血压病社区综合性干预的效果评价

摘　　要：目的探讨社区综合性干预对原发性高血压患者的知识、态度、行为(KAP)及血压控制的作用。方法选择武汉市东西湖区常青花园社区卫生服务中心慢性病重点人群管理册中的原发性高血压患者402例,随机分为干预组206例和对照组196例,干预组给予综合性干预,对照组仅给予一般健康干预措施。在干预前后进行调查,对干预过程和结果进行评价。结果对照组干预后KAP得分仅认知水平K发生明显的变化,较干预前差异有统计学意义(P<0.05);干预前后收缩压和舒张压比较差异均无统计学意义(P>0.05)。干预组干预后,KAP得分分别由干预前的(13.45±5.46)分、(18.11±2.82)分、(21.84±3.70)分提高到干预后的(21.00±3.46)分、(25.09±2.18)分、(26.25±2.60)分,差异均有统计学意义(P<0.05);干预组干预前的平均收缩压为(156.60±13.40)mmHg、舒张压为(97.29±10.21)mmHg,干预后平均收缩压为(135.16±9.84)mmHg,舒张压为(84.36±8.14)mmHg,差异均有统计学意义(P<0.05)。结论综合性干预是社区原发性高血压防治的重要措施和有效途径。Objective To investigate the effect of comprehensive intervention on knowledge,attitude,practices （KAP） and controlling blood pressure of patients with essential hypertension in community.Methods Totally 402 patients with essential hypertension,chosen from key population enrollment of chronic diseases in Changqing Garden community health service centre from Dongxihu district in Wuhan city,were divided as trial group （n=206） and control group （n=196）.The patients in the trial group were given comprehensive intervention,and those in the control group,conventional intervention.The investigation was conducted before and after the intervention,and the intervention process and the results were assessed.Results After intervention,in the control group only the knowledge level significantly increased （P〈0.05）.However,in the trial group the KAP scores increased respectively from （13.45±5.46）,（18.11±2.82） and （21.84±3.70） to （21.00±3.46）,（25.09±2.18） and （26.25±2.60） （P〈0.05）;the average systolic blood pressure （SBP） decreased from （156.60±13.40） mmHg to （135.16±9.84） mmHg,the average diastolic blood pressure （DBP） decreased from （97.29±10.21） mmHg to （84.36±8.14） mmHg （P〈0.05）.Conclusion The comprehensive intervention is important and effective for control of essential hypertension in community.
