糖心平合剂对糖尿病心脏自主神经病变心率变异指标的影响

摘　　要：目的:观察糖心平合剂对糖尿病心脏自主神经病变的临床疗效及对心电图正常心动周期标准差(SDNN)、心率变异指数(HRVI)、功率谱的影响,探讨糖心平合剂治疗糖尿病心脏自主神经病变的作用机理。方法:61例患者分为对照组(29名)和糖心平合剂治疗组(32名),观察临床治疗效果,检测治疗前后对SDNN、HRVI和功率谱的影响。结果:治疗组和对照组共60例患者,治疗2周后,在中医证候方面治疗组较对照组有明显优势(P<0.01),治疗8周后,仍较对照组有优势(P<0.05);治疗4周后治疗组可见糖化血红蛋白较治疗前有所改善(P<0.05),而对照组前后对比无明显差别;治疗组在心电图功率谱、心率变异指数方面均较治疗前有显著改变(P<0.01),而对照组仅在功率谱上体现出疗效(P<0.05)。结论:糖心平在治疗糖尿病心脏自主神经病变方面,通过改善临床症状体征,减轻患者心率变异程度,稳定血糖,改善尿糖、尿蛋白含量等手段,取得肯定疗效。objective：To observe the curative influences of Tangxinping on the Diabetic Mellitus（DM）and its influences on the SDNN,Heart Rate Variability Index（HRVI）and power spectrum.To explore the mechanism of action about Tangxinping treating the DM cardiovascular autonomic neuropathy.Methods：61 cases of patients were randomly divided into the control group and the Tangxinping treatment group,29 cases of control group and 32 cases of treatment group.The clinical HRVI and power spectrum prior and post treatment were detected.Results：After two weeks＇ treatment,clinical symptoms and signs of treatment group were significantly different compared with the control group（P〈0.01）.After 8 weeks＇ treatment,there was still quite different.After 4 weeks of of treatment,the glycosylated hemoglobin of treatment group were greatly improved during the treatment（P〈0.01）,meanwhile there was no difference about the ones in the control group in the course of treatment.The ECG power spectrum and HRVI of treatment group after treatment were significantly different compared with the ones before treatment（P〈0.05）,whereas only ECG power spectrum were greatly changed in the control group during the period of treatment（P〈0.05）.Conclusion：Through improving the clinical symptoms,easing the patients＇ HRVI,stabilizing blood sugar,ameliorating urine glucose and urine protein,Tangxinping can treat DM cardiovascular autonomic neuropathy.
