室温生理盐水胃冲洗应用于应激性溃疡的止血效果观察

摘　　要：目的探讨室温胃冲洗液对应激性溃疡上消化道出血的治疗效果。方法将79例外科重症及大手术后应激性溃疡出血患者随机分为对照组(39例)和观察组(40例),对照组采用冷生理盐水(4～10℃)、观察组采用室温生理盐水(22～24℃)进行胃冲洗及药物胃灌注治疗,比较两组止血效果和再出血发生率。结果观察组止血效果显著优于对照组(P<0.05),再出血发生率显著低于对照组(P<0.01)。结论室温生理盐水胃冲洗和灌注治疗应激性溃疡,仅引起胃表面血管收缩,达到止血作用,对全身影响较小,利于胃黏膜修复,其效果优于冷生理盐水。Objective To probe into the effect of gastric irrigation with room temperature saline on upper gastrointestinal bleeding induced by stress ulcers. Methods Totally, 79 patients who had underwent a major surgery and developed upper gastrointestinal bleeding due to stress ulcer were randomly divided into a control group （n= 39） and an observation group （n= 40）. Patients in the control group used cold saline （4 -10℃） and the observation group patients used room temperature saline（22 -24℃） for gastric flushing and irrigation. The haemostatic effect and rates of rebleeding of the two groups were compared. Results The haemostatic effect of the observation group were better than that of the control group （P〈0.05）, and rates of rebleeding in the observation group were significantly lower than that in the control group （P〈0. 01）. Conclusion Flushing and irrigation with room temperature saline for stress ulcer helps the gastric superficial vessels to constrict and obtains more excellent haemostatic effect than cold saline. In this way, the blood circulation of the whole body is not affected, and the gastric mucosa is able to repair.
