摘　　要：目的:探讨轻度肾功能不全与C-反应蛋白(CRP)及冠状动脉斑块形态的关系。方法:对100例冠心病(CHD)患者行冠脉造影(CAG),按入院时血清肌酐(Cr)是否>106μmol/L分为肾功能正常组及轻度肾功能不全组,根据CAG的冠脉粥样斑块形态学分型分为Ⅰ型、Ⅱ型、Ⅲ型,分析轻度肾功能不全与CRP及冠脉斑块形态的关系。结果:血清CRP在轻度肾功能不全组明显增高(P<0.05),血清Cr和CRP正相关;Ⅱ型亚组Cr和CRP水平最高,Ⅲ型、Ⅰ型亚组依次减低,组内比较差异有统计学意义(P<0.05)。结论:反映斑块不稳定的Ⅱ型斑块的血清Cr水平明显增高,提示轻度肾功能不全可作为评估CHD严重程度的参考指标之一。Objective： To evaluate the relationship between mild renal dysfunction and c--reaction protein（CRP）,and that between mild renal dysfunction and angiographic morphology of coronary lesions. Methods： 100 cases of coronary heart disease （CHD） with coronary angiography （CAG）were divided into the normal renal function group（CrY106 μmol/L）and the mild renal dysfunction group（CrY106 /amol/L）according to serum creatinine（Cr）. The patients in the mild renal dysfunction group were subdivided into three groups according to the morphological division of the plaque （type I lesion, typeIIIlesion and type III lesion） to analyze the relationship between mild renal dysfunction and CRP and that between mild renal dysfunction and angiographic morphology of coronary lesions. Results： serum CRP concentration significantly increased in mild renal dysfunction group compared with normal renal function group（ P〈0. 05）. Serum Cr level was positively correlated with the CRP concentration. The levels of serum Cr and CRP in type IIplaque group were the highest, and then the levels in type III plaque group, with those in type Iplaque group the lowest. Differences were statistically significant among Subgroups（ P 〈 0. 05）. Conclusion： The level of serum Cr in type II plaque group as the rupture of the plaque is significantly raised ,which suggests that mild renal dysfunction can be a reference for evaluating the severity of CHD.
