结膜细胞外基质与羊膜在结膜重建中的实验研究

摘　　要：目的:观察结膜细胞外基质(extracellular matrix,ECM)与羊膜作为结膜组织的替代材料用于结膜成形的特点及效果。方法:饲养30只新西兰白兔,将其中6只白兔的球结膜制成结膜ECM,并将剩余的白兔随机分为实验组和对照组,每组12只;实验组兔结膜采用组织工程技术制备的结膜ECM行结膜移植修补术,对照组采用低温冷冻保存的羊膜移植,以观察移植修补术后结膜成形的情况,并行肉眼、镜下、免疫组化及淋巴细胞毒检查。结果:实验组结膜ECM移植后1周可见新生血管自移植边缘长入,8周时外观已接近正常结膜,无免疫排斥反应,光镜下可见移植后4周的移植区上皮已接近正常结膜上皮;对照组术后4周移植区仍有部分上皮未覆盖。结论:结膜ECM为目前比较理想的结膜替代材料,并可有效地用于角膜、结膜表面重建。Objective： To investigate the characteristics and the effect of conjunetival extracellular matrix （ECM） and Amniotic membrane as a succedaneum for conjunctiva. Methods： 30 New Zealand rabbits were raised for the experiment. The conjunctival defect models were made of bulber conjunctiva from six rabbits and then all others were randomly divided into the test group（12 Rabbits） and the control group（12 rabbits）. The conjunctival defect in the test group yeas repaired with conjunctival ECM and that in the control group was repaired with transplantation of the refrigerated amniotic membrane. All of them were photographed and examined by light microscope , immunohistochemical examination and lymphocyte toxicity test. Results： There were blood vessels growing into the grafts one week after conjunctival ECM transplantation in the test group and the grafts became mildly congestive and similar to normal conjunctival ones in the 8th week, without any immunologic rejection. Epithelium in the grafts could be seen through the light microscope to grow close to the normal one 4 weeks after transplation. But in the control group, the grafts were not completely covered with epithelium 4 weeks after transplantation, Onclusion： Conjunctival ECM has the characteristics of tissue activity and low antigenicity. It is an ideal conjunctival succedaneum for conjunctiva in conjunctiva transplantation and could be used as a graft to reconstruct ocular surface as well.
