摘 要:目的:检测子宫内膜异位症患者血清中基质金属蛋白酶-9(MMP-9)的含量,探讨MMP-9在子宫内膜异位症发病中的作用及意义。方法:收集子宫内膜异位症患者血清标本35例,其中增生期24例,分泌期11例;按美国生育协会(R-AFS)分期标准分期,Ⅰ~Ⅱ期12例、Ⅲ~Ⅳ期23例;患者的临床病理类型有卵巢型20例,腹膜型4例,深部结节型11例。对照组为子宫肌瘤患者,共25例,收集其手术前血清标本,其中增生期11例,分泌期14例。应用酶联免疫吸附试验法(ELISA)检测子宫内膜异位症(EMS)血清中MMP-9的表达,同时检测子宫肌瘤患者血清中MMP-9表达,分析两者是否存在差异性。结果:①子宫内膜异位症患者与对照组血清中MMP-9的水平差异有统计学意义(P<0.05);②子宫内膜异位症患者按R-AFS分期Ⅲ~Ⅳ期血清中MMP-9含量与Ⅰ~Ⅱ期MMP-9含量差异有统计学意义(P<0.05);③子宫内膜异位症患者血清MMP-9的含量与月经周期无明显关系(P>0.05);④EMS腹膜型、深部结节型、卵巢型的血清中MMP-9的含量差异有统计学意义(P<0.05)。结论:①MMP-9可能与子宫内膜异位症发病及病情进展有关,是EMS发病及病情进展的因素之一;②MMP-9在EMS的高表达为MMP-9特异性抑制剂对EMS的治疗提供了可能的理论依据。Objective : To detect the level of serum matrix metalloproteinases - 9 ( MMP - 9) in patients with endometriosis, explore the effect and significance of MMP -9 in endometriosis. Methods: Serum samples of 35 patients with endometriosis were collected, in-cluding 24 samples in proliferative phase and 11 samples in secretion phase ; according to the standard of American Fertility Society ( R -AFS), 35 patients were divided into stage Ⅰ-Ⅱ ( 12 patients) and stage Ⅲ-Ⅳ (23 patients) ; according to pathological types, 35 patients were divided into ovarian endometriosis (20 patients), peritoneal endometriosis (4 patients) and deep infiltrating endometriosis ( 11 patients) . 25 patients with hysteromyoma were selected as control group, the preoperative serum samples were collected, including 11 samples in proliferative phase and 14 samples in secretion phase. The expression levels of serum MMP - 9 in endometriosis and hysteromyoma were detected by ELISA, the difference was analyzed. Results: There was significant difference in serum MMP - 9 level between the two groups ( P 〈 0.05 ) ; in endometriosis group, there was significant difference in serum MMP - 9 level between stage Ⅲ-Ⅳ and stage Ⅰ-Ⅱ ( P 〈 0. 05 ) ; there was no correlation between serum MMP - 9 level in endometriosis and menstrual cycle ( P 〉 0. 05 ) ; in endometriosis group, there was significant difference in serum MMP -9 level among ovarian endometriosis, peritoneal endometriosis and deep infiltrating endometriosis ( P 〈 0. 05 ) . Conclusion: MMP - 9 may play an important role in pathogenesis and development of endometriosis, and it may be an important factor of oneogenesis and development of endometriosis; high expression of MMP- 9 in endometriosis provides a theoretical basis for anti - MMP -9 therapy in treatment of endometriosis.
