三维超声造影评价肾肿瘤的应用研究

摘　　要：目的三维超声造影(three-dimensional contrast-enhanced ultrasound,3D-CEUS)是一种新型成像技术,可用于评估肿瘤的血供、内部结构及其边界。探讨3D-CEUS在肾肿瘤中的应用价值。方法对35例肾肿瘤患者行三维彩色血管能量成像(three-dimensional color power angiography,3D-CPA),二维超声造影(two-dimensional contrast-enhanced ultrasound,2D-CEUS)检查后再行3D-CEUS检查。重建三维图像后利用后处理软件观察病灶的血管特征,对比2种三维成像技术对肾肿瘤滋养血管的显示率。结果3D-CEUS成像时,皮质相早期采集获知病灶的滋养动脉及起始强化部位,皮质相肾皮质强化后采集显示瘤灶的强化程度及方式,延迟相采集清晰显示瘤灶境界、坏死灶范围。3D-CEUS/3D-CPA对同组肾肿瘤滋养血管的显示率为82.86%/51.43%。结论3D-CEUS可有效评估肾肿瘤的血供情况,为临床提供更丰富的信息。Objective Three-dimensional contrast-enhanced ultrasound（3D-CEUS） is a new imaging technique which can easily display abnormal vascular lesions,internal structures and the surrounding tissues.This study aimed to explore the clinical diagnostic value of 3D-CEUS in renal tumor.Methods 3D-CEUS was taken on the foundation of three-dimensional color power energy imaging（3D-CPA） and 2D-CEUS in 35 patients with renal tumors.3-D imaging was reconstructed and the vascular characteristics of tumors were observed by the use of post-processing software on the workstation.The detected rate of tumor vascular texture was compared between 3D-CEUS and 3D-CPA.Results Vascular texture and initial enhanced spot could be obersverd in the early cortex phase of 3D-CEUS imagings,then the enhancement extent and patterns of neoplasms could be displayed,soon after the boundaries and necrosis could be limpid showed in the delayed phase.The detected rate of 3D-CEUS and 3D-CPA in tumor vascular texture were 82.86% and 51.43%.Conclusion 3D-CEUS can display the blood supply feature of renal neoplasms sensitively and provide more information to the clinial diagnosis.
