下肢骨折患者血浆中纤维蛋白原和D-二聚体检测结果分析

摘　　要：目的探讨血浆中纤维蛋白原(FIB)及D-二聚体(D-dimerDD)检测对下肢骨折并发深静脉血栓(DVT)的早期诊断价值。方法对90例下肢骨折患者术前及术后第3天进行血浆中FIB及DD的定量检测,并与正常对照组(90例)比较。结果骨折患者术前术后3d的血浆中FIB和DD均高于对照组(P<0.01);90例患者有10例并发DVT,并发DVT组术前、术后第3天血浆中FIB及DD的检测结果差异有统计学意义(P<0.05)。未并发DVT组术前、术后第3天血浆中FIB及DD的检测结果差异无统计学意义(P>0.05)。而并发DVT组与未并发DVT组相比较,术前血浆中FIB及DD的检测结果差异无统计学意义(P>0.05);术后第3天血浆中FIB及DD的检测结果差异有统计学意义(P<0.01)。结论血浆中FIB及DD的动态监测对下肢骨折并发DVT具有早期诊断价值。[Objective]To discuss the early diagnostic value of plasma fibrinogen（FIB） and D-dimer assay on patients with lower limb fracture and deep vein thrombosis （DVT）.[Methods]Plasma FIB and DD were quantitatively detected on 90 patients with lower limb fracture preoperatively and 3 days postoperatively.The results were compared with control group.[Results]Preoperative and 3 days postoperative plasma FIB and DD levels of limb fracture patients were significantly higher than those of control group （P〈0.01）.Of 90 patients,10 combined with DVT,the difference of FIB and DD levels were significant before and 3 days after operation（P〈0.05）;for patients without DVT,there was no significant difference （P〉0.05）.Comparing DVT patients with non-DVT patients,the testing results of FID and DD were not significantly different before operation（P〉0.05）,but significantly different 3 days after operation（P〈0.01）.[Conclusion]Dynamic assay of FIB and DD has value in early diagnosis of （DVT） after lower limb fracture.
