摘  要:目的探讨核因子-κB(NF-κB)和Th1/Th2免疫失衡在IgA肾病(IgAN)发病中的作用及黄芪防治IgAN的机制。方法32只SD大鼠分为模型组、干预组和对照组。模型组和干预组复制IgAN模型,干预组予黄芪颗粒剂混悬液口服,并以正常SD大鼠作为对照组。检测各组大鼠蛋白尿、血尿及肾脏病理改变,采用免疫组织化学技术检测各组大鼠肾组织NF-κB、转化生长因子-β1(TGF-β1)、单核细胞趋化蛋白-1(MCP-1)的表达;ELISA法检测其血清中Th1类细胞因子INF-γ和Th2类细胞因子IL-4水平。结果1.模型组大鼠尿红细胞、尿蛋白水平较对照组明显增高,差异有统计学意义(Pa<0.01);2.模型组大鼠肾组织NF-κB、MCP-1、TGF-β1的表达与干预组、对照组比较均明显增高,差异均有统计学意义(Pa<0.01);3.模型组大鼠血清INF-γ水平与干预组、对照组比较均显著下降(Pa<0.05),IL-4水平与干预组、对照组比较均显著升高(Pa<0.01);4.模型组大鼠肾小球系膜区、肾小管、肾间质病理损害较干预组和对照组加重。结论黄芪能降低IgAN大鼠尿红细胞数、尿蛋白水平,并减轻IgAN模型大鼠肾脏病理损害,可能的机制是调节Th1、Th2平衡紊乱及抑制NF-κB激活,并减少大鼠肾组织MCP-1、TGF-β1的表达,延缓其IgAN的发生发展。Objective To explore the role of nuclear factor - κB (NF - κB) and Th1/Th2 immune imbalance in experimental IgA nephropathy(IgAN) rat model pathogenesis, and to explore the mechanism of astragalus on IgAN. Methods Thirty -two SD rats were divided into IgAN model group,treament group and normal control group. IgAN rats were treated with or without astragalus orally. Rats in normal control group were treated with water instead of astragalus orally. The rats were examined with albuminuria, haematuria and pathological change of renal tissue and examined the distribution of transforming growth factor - β1 (TGF - β1 ), monocyte chemoactive peptide 1 (MCP - 1 ) and NF - κB in renal tissue by immunohistochemistry technology and interferon (INF -γ),interleukin -4 (IL- 4) of Th1 and Th2 were detected in IgAN rat model by Sandwich enzyme linked immunosorbent assay(ELISA). Results 1. The albuminuria, haematuria in IgAN model group decreased remarkably compared with those in normal control group( Pa 〈 0.01 ). 2. Expressions of the NF - κB, MCP - 1 and TGF - β1 in renal tissues IgAN model group were significantly highter than those in treatment group and normal control group(P 〈 0.01 ). 3. The level of serum INF-γ in IgAN model group were descended compared with treatment group and normal control group(Pa 〈 0.05 ) ,the serum IL-4 in IgAN model group was ascended obviously compared with treatment group and normal control group( Pa 〈0.01 ). 4. The injury of glomerular mesangium, nephric tubule and renal interstitium pathology were compared gravely than treament group and normal control group. Conclusions Th2 dominant reaction and the activity of NF - κB plays an important role in the pathogenesis of IgAN. Astragalus can decrease albuminuria, haematuria and pathological change of renal tissue ,may be associated with the regulate the Th1/Th2 immune imbalance and down regulation of NF - κB ,MCP - 1 ,TGF - β1 expressions ,delay the occurrence and development of IgAN.
