摘 要:目的测试梯形钛网植入后植骨界面的生物力学性能,为临床颈椎前路单椎体次全切术中应用梯形钛网提供依据。方法骨密度无差异的12具颈椎标本,术前拍摄颈椎侧位X线片作为空白对照组(C组),随机分为直形钛网组(B组)和梯形钛网组(A组)各6具,测量颈椎弧度、椎间角度、椎间高度、手术间隙缩窄1 mm的最大载荷、刚度。结果颈椎前凸弧度和手术节段椎间角度在A组与B、C组比较有统计学差异(P<0.01),B、C组间亦有统计学差异(P<0.05);手术节段椎间高度在A、B组与C组比较有统计学差异(P<0.01)。A、B组手术间隙缩窄1 mm的最大载荷及刚度比较有统计学差异(P<0.05)。结论经修剪成梯形的钛网不但更适配椎间隙前高后低的形态特征,更具备良好的生物力学性能。Objective To test biomechanical properties of bone interface after trapezoidal titanium mesh implants and to provide a basis for the application of trapezoidal titanium mesh in clinical anterior cervical vertebrae subtotal resection of single segment. Methods A total of 12 cervical specimens without difference in bone mineral density and lateral cervical X-ray before operation were taken as control group ( group C ) , then the 12 specimens were randomly divided into two groups that was straight-shaped titanium mesh group (group B) and trapezoidal titanium mesh group (group A). The cervical curvature, intervertebral angle, intervertebral, maximum load and stiffness to constrict the operation segment intervertebral clearance for 1 mm were measured. Results There was a significant difference on cervical lordosis angle and intervertebral angle of surgical segment between group A and group B, C ( P 〈 0. 01 ) , as well as a significant difference between group B and C ( P 〈 0.05 ). There was also a significant difference on intervertebral height of surgical segment between group A, B and group C ( P 〈 0. 01 ). There was a significant difference on the degree of maximum load and stiffness to constrict the operation segment intervertebral clearance for 1 mm between group A and B ( P 〈0. 05 ). Conclusion Trapezoidal titanium is more fit for the intervertebral shape from high to low and have a better biomechanical properties.
