农村育龄妇女分娩情况分析与评价

摘　　要：目的探讨项目县合作医疗制度实施前后农村育龄妇女分娩情况的变化趋势及影响因素。方法根据流行病学干预对照原理，应用SPSS 17.0统计软件进行描述性分析和Logistic回归分析项目县4个样本乡镇1997~2006年农村育龄妇女的分娩情况。结果 ①10年内居住于样本乡镇的妇女选择在县级医疗机构分娩的人数少于乡镇卫生院，县级医疗机构分娩人数占总人数的23.06%；②10年内样本乡镇新生儿体重变化趋势不明显，县级医疗机构低出生体重婴儿出现的可能性是乡镇卫生院的2倍；③剖宫产率逐年增高，10年内样本乡镇卫生院的总剖宫产率已达28.74%，预测胎儿体重较重和首胎婴儿更趋向于以剖宫产的方式出生。结论合作医疗制度未对农村育龄妇女分娩情况产生影响，新生儿体重与地区经济水平有关，剖宫产率与地区经济水平、巨大儿和胎次有关。Objectives To analyze trends in delivery and its influencing factors of rural women of childbearing age after implementation of cooperative medical care policy in rural areas of MCH project counties. Methods According to intervention contrast principle, descriptive study and logistic analysis for delivery situation of women in 4 sample townships of MCH project counties over a period from 1997 to 2006 were conducted with SPSS 17.0 statistic software. Results ①The proportion of rural women who delivered in the countylevel hospitals （only accounting for 23.06%） was less than that of women who delivered in the township health centers in the 10year period;②The change trend in newborn’s birth weight was not significant, inversely, the rate of low birth weight infants born in the countylevel hospitals was twice of that in the township health centers;③Cesarean section rate was increased year by year and the total cesarean section rate in townshiplevel health centers had reached to 28.74%. The women with heavier estemated infants and primiparae were more likely to deliver their baby under cesarean section. Conclusion The cooperative medical care system doesn’t affect delivery situation of rural women. The birth weight of the infant is related to regional economic level.Cesarean section rate is related to regional economic level, macrosomia and parity.
