摘 要:目的分析妊娠期高血压疾病孕产妇及其新生儿常规脑电图的特点。方法 对2008年6月至2009年4月在汕头市妇女儿童医院产科住院分娩的妊娠期高血压疾病孕产妇及其新生儿进行脑电图检查,分析其特点并与正常孕产妇及其新生儿脑电图进行比较。结果①23例正常分娩孕产妇的脑电图均为正常。35例妊娠期高血压疾病孕产妇(子痫前期32例;子痫1例;慢性高血压合并轻、重度子痫前期各1例)中,正常脑电图为16例,异常脑电图为19例,其中15例表现为癫痫样放电,2例以慢波活动为主,2例以快波活动为主。妊娠期高血压疾病孕产妇与正常孕产妇的脑电图结果经比较有非常显著性差异(χ2=18.57,P〈0.01);②正常孕产妇所生新生儿23例,脑电图异常者1例。妊娠期高血压疾病孕产妇所生新生儿进行脑电图检查者为30例,其中脑电图正常者26例、异常者4例,经Fisher精确检验,两者无显著性差异(P=0.37)。结论①妊娠期高血压疾病孕产妇脑电图改变以癫痫样放电为主,与正常孕产妇相比,妊娠期高血压疾病孕产妇的脑电图变化显著;②孕产妇患妊娠期高血压疾病对新生儿的脑电图可能无明显影响。Objective To study electroencephalogram(EEG) features of women with hypertensive disorders of pregnancy(HDP) and their newborns. Methods 35 pregnant women with HDP and 23 normal pregnant women and their newborns in Department of Obstetrics, Shantou Municipal Women and Children’s Hospital were examined with EEG in the ward from June 2008 to April 2009. The EEG features of pregnant women with HDP were studied and compared with those of normal pregnant women in the same period. Meanwhile, the EEGs of the newborns whose mothers suffered from HDP were compared with those whose mothers had normal pregnancy. Results ①All the EEGs of 23 women with normal pregnancy were normal. Among 35 women with HDP( including 32 women with preeclampsia, 1 woman with eclampsia, 2 women with chronic hypertension associated with mild or severe preeclampsia), 16 women’s EEGs were normal and 19 women’s EEGs were abnormal( of which, 15 women’s EEGs shown epileptiform discharges, 2 women’s EEGs shown fast waves and 2 women’s EEGs shown slow waves). In EEG result, there was a significant difference between women with HDP and normal pregnant women(χ2=18.57, P〈0.01); ②There were 23 newborns born by normal pregnant women and 22 newborns’ EEG were normal except 1 abnormal EEG. There were 30 newborns born by women with HDP, of which, 26 newborns had normal EEG and 4 newborns had abnormal EEG. By Fisher’s test, there was no significant difference between the newborns born by women with HDP and those born by normal pregnant women(P=0.37〉0.05).Conclusion ①Epileptiform discharge is a main EEG feature of women with HDP. Compared with normal pregnant women, the changs in EEGs of those pregnant women with HDP are more significant; ②HDP of the mother doesn’t effect on EEG of the newborns.
