摘  要:目的评价高碘酸钠-氧化低分子肝素-抗凝血酶共价复合物(SPLMWATH)涂层体外循环医用聚氯乙烯(PVC)管道的生物相容性。方法利用蛋白非酶糖基化反应合成SPLMWATH,应用聚乙烯亚胺-戊二醛结合技术分别制备SPLMWATH涂层、肝素涂层和低分子肝素涂层PVC管道。将PVC管道分为四组:A组(无涂层)、B组(肝素涂层)、C组(低分子肝素涂层)和D组(SPLMWATH涂层)。人抗凝全血用Chandlar loop模型体外模拟转流4h后,分别测定各组的血小板计数、纤维蛋白原含量及血浆白介素(IL)-8和单核细胞趋化蛋白-1(MCP-1)的浓度,并用扫描电镜观察各组管道的内表面情况。结果各组血小板计数和纤维蛋白原含量均较基线值有所下降,其中D组下降程度最小,与B组和C组间有显著性差异(P<0.05),且B组和C组之间也有显著性差异(P<0.05)。各组血浆IL-8和MCP-1的浓度均较基线值有所升高,其中D组升高程度最小,与B组和C组间有显著性差异(P<0.05),而B组和C组之间无显著性差异(P>0.05)。扫描电镜观察结果显示B组、C组和D组与A组比较均减少了血小板和纤维蛋白粘附,其中D组管道的表面情况明显优于B组和C组,C组略优于B组。结论①涂层处理过的PVC管道减少了血液相关成分的激活和消耗;②SPLMWATH涂层PVC管道具有较肝素涂层和低分子肝素涂层管道更佳的生物相容性,减轻了血液与PVC管道表面接触所引起的炎性介质生成、释放和凝血系统激活。OBJECTIVE To evaluate the biocompatibility of sodium periodate - oxidated low molecular weight heparin - antithrombin covalent complex (SPLMWATH) coated polyvinyl chloride (PVC) tubing. METHODS Basing on non - enzymatic protein glycated reaction, the SPLMWATH was produced. By using polyethyleneimine - glutaraldehyde bonding technique, PVC tubing was coated with SPLMWATH,heparin and low molecular weight heparin (LMWH). PVC tubing was divided into four groups: group A ( non - coated), group B ( heparin - coated), group C ( LMWH - coated) and group D ( SPLMWATH - coated). Human anticoagulant whole blood was incubated in rotating PVC tubing for 4 hours, using Chandlar loop model. The platelet count, content of fibrinogen in plasma and plasma concentrations of interleukin(IL) -8 and monocyte chemoattractant protein 1 (MCP- 1 ) were quantified. Scan- ning electron microscope(SEM) was used to observe the internal surface of each group tubing. RESULTS The platelet count and content of fibrinogen in plasma were both decreased in all groups from their baseline values, and the decreasing degree of group D were minimal with significant differences compared with that of group B or group C ( P 〈 0.05 ). Also, there were significant differeneas between group B and group C ( P 〈 0.05 ). Plasma concentrations of the IL - 8 and MCP - 1 were both increased in all groups from their baseline values, and the increasing degree of group D were minimal with significant differences compared with that of group B or group C( P 〈0.05). But there were no significant differences between group B and group C( P 〉0.05). Results of SEM showed group B,group C and group D can reduce the adhesion of platelets and fibrin when compared with group A. The outcome of group D was much better than that of group B or group C, and group C was slightly superior to group B. CONCLUSION I. Coated - PVC tubing can reduce activation and consumption of blood - related components; 2. SPLMWATH - coated
