抗痨凝胶介入治疗对异体肾移植术后并发肺结核空洞病人的临床分析

摘　　要：目的:探讨抗痨凝胶介入治疗对异体肾移植术后病人并发空洞肺结核的近远期疗效。方法:将异体肾移植术后并发空洞肺结核的病人随机分成研究组(32例)及对照组(28例)。治疗组经纤维支气管镜灌注含药凝胶治疗和用抗结核药物治疗;对照组仅用抗结核药物治疗。观察各组病人治疗6个月后的痰菌转阴率和空洞闭合率,随访2年,观察各组病人的痰菌复阳率。结果:治疗6个月后治疗组和对照组的痰菌转阴率分别为93.5%和70.4%(P<0.05),空洞闭合率分别为87.1%和59.3%(P<0.05)。随访2年后,治疗组和对照组痰菌复阳率分别为6.7%和32.0%(P<0.05)。治疗组无严重不良反应。结论:抗痨凝胶介入治疗对异体肾移植术后病人并发空洞肺结核的治疗近、远期疗效确切,痰菌复阳率低,是一种安全有效的治疗方法。Objective：To analyse the short-term results and long-term results of renal transplantation patients presented with cavity in pulmonary by antiphthisic gel in the intervention treatment.Methods：The renal transplantation patients presented with cavity in pulmonary patients were divided into two groups：32 cases in treatment group and 28 cases in control group.The patients of treatment group were treated with containing drug gel by bronchofiberscope and medication,the patients of control group were treated only with medication.At the end 6 month therapy, the sputum negative conversion rate and the cavity curative effect rate of the patients were observed.At the final follow up after 2 yuars, the sputum positive conversion rate was observed.Results：At the end 6 month therapy, the sputum negative conversion rate of the treatment group patients and the control group patients was 93.5% and 70.4% respectively（P〈0.05）,the cavity curative effect rate of the treatment group patients and the control group patients was 87.1% and 59.3% respectively（P〈0.05）.At the final follow up after 2 yuars, the sputum positive conversion rate of the treatment group patients and the control group patients was 6.7% and 32.0% respectively（P〈0.05）.There were not severe adverse reaction in treatment group.Conclusion：Intervention therapy is a effective treatment for renal transplantation patients presented with cavity in pulmonary in short-term and long-term.This treatment plays a important role in reducing long-term relapse and is very safe.
