左炔诺孕酮宫内缓释系统治疗围绝经期功血的疗效观察

摘　　要：目的:观察左炔诺孕酮宫内缓释系统((levonorgestrel intrauterine system,LNG-IUS,商品名:曼月乐)对围绝经期功能失调性子宫出血(围绝经期功血)的临床疗效。方法:对39例子宫内膜诊刮诊断为功能失调性子宫出血患者放置曼月乐,在放置前和放置后12个月内定期B超监测子宫内膜厚度,观察患者血红蛋白情况及月经改善情况。结果:置宫内节育器3个月后,子宫内膜厚度由(12.5±6.3)mm降至(7.5±2.3)mm,血红蛋白由(88.46±11.3)g/L上升至(115.4±7.5)g/L,月经量明显减少,由置宫内节育器前(164.9+41.5)ml降至(52.2±13.8)ml,置宫内节育器前后两者比较差异具有统计学意义。结论:曼月乐治疗围绝经期功能失调性子宫出血疗效好,值得临床推广。Objective：To observe the clinical effectiveness of Levonorgestrel intrauterine system （Mirena） in treating dysfunctional uterine bleeding during peri-menopausal period.Methods：39 patients,diagnosed with dysfunctional uterine bleeding by dilation and curettage,were placed with Mirenas.The haemoglobin and menstruati Dn of these patients were observed and the endometrium width was examined via B ultrasound both before and after the placement.Results：3 tnonths later after the placement of the devices,the haemoglobin of these patients was increased from （88.46±1.33 to （115.4±7.5）g/L with significant decrease of the menstrual volume from （164.9±41,5） to （52.2 ±13.8） ml and the reduction of the endometrium width from （12.5±6.3）to （7.5±2.3） mm.Conelusion：Mirena is worthy of wide utilization in clinic due to its good effectiveness in treating dysfunctional uterine bleeding during peri-menopausal period.
