胰导管内灌注含胰酶抑制剂的UW液对胰导管保护效果的评价

摘　　要：目的:评价胰导管保护法(即在胰腺获取时向胰导管内注射少量含胰酶抑制剂trypsin inhibitor的UW液,TIUW液)对冷保存期间胰导管的保护效果。方法:大鼠胰腺分为5组。组1,新鲜胰腺(n=10);组2,未应用胰导管保护法仅用TIUW液保存6h的胰腺(n=10);组3,应用胰导管保护法且用TIUW液保存6h的胰腺(n=10);组4,未应用胰导管保护法仅用TIUW液保存24h的胰腺(n=10);组5,应用胰导管保护法且用TIUW液保存24h的胰腺(n=10);应用锥虫蓝染色评价胰导管细胞的死亡率,石蜡切片观察胰导管保护后胶原酶的分布情况。结果:胰导管细胞死亡率分别为(5.5±3.0)%、(58.6±9.1)%、(16.8±4.9)%、(64.4±8.1)%、(17.2±3.0)%,组2和组3相比、组4和组5相比差异均有统计学意义(P<0.05),组3和组5相比差异无统计学意义(P>0.05)。应用胰导管保护法组的胶原酶分布均较未应用组的均匀。结论:胰导管保护法较好的保存了胰导管系统的完整性,且胰导管的活性状态不随时间推移而恶化;胰导管系统的完整性是胶原酶均匀分布的前提。Objective To explore the protective effect of intraductal injections of University of Wisconsin solution containing a small amount of trypsin inhibitor （TIUW solution） on the pancreatic duct under cryopreservation. Methods The rat pancreata were divided into five groups： fresh pancrcata （group 1, n = 10）, and pancreata preserved in TIUW solution for 6 or 24 hours without or with intraductal infusions of the same solution （groups 2, 3, 4, and 5, respectively; n = 10 for each）. The rate of cell death in the pancreatic duets was measured by trypan blue uptake, and the distribution of collagenase in paraffin sections was observed. Results The rate of cell death was （5.5 ±3.0）% in group 1, （58.6 ± 9.1）% in group 2, （16.8 ± 4.9）% in group 3, （64.4 ± 8.1）% in group 4, and （17.2 ±3.0）% in group 5; it differed significantly between group 2 and group 3 or between group 4 and group 5 （P 〈 0.05 for both comparisons） but not between group 3 and group 5 （P 〉 0.05）. The coilageuase distributed more evenly in groups 3 and 5 than in groups 2 and 4. Conclusions Intraductal injections of TIUW solution can save the integrity of the pancreatic duct system which has been the basic of the even distribution of eollagenase. The viability of the pancreatic ducts do not weaken with time.
