摘  要:目的:探讨代谢综合征(MS)患者在不同时期血清肿瘤坏死因了-α(TNF-α)、白介素-6(IL-6)浓度变化的意义以及TNF-α、IL-6与胰岛素抵抗(IR)的相互关系。方法:90例老年肥胖患者根据糖脂代谢紊乱程度分为代谢综合征合并2型糖尿病(T2DM)组、肥胖合并T2DM组、单纯肥胖组各30例。30例健康者为健康对照组。所有受试者均采用酶联免疫吸附法检测血清TNF-α、IL-6水平。结果:①肥胖各组TNF-α、IL-6均较健康对照组明显升高(P<0.01),且TNF-α、IL-6在单纯肥胖组、肥胖合并T2DM组、MS合并T2DM组呈逐渐升高,组间比较差异有统计学意义(P<0.01)。②血清TNF-α、IL-6与稳态模型胰岛素抵抗指数(HOMA-IR)呈正相关关系(P<0.01)。结论:代谢综合征患者血清TNF-α、IL-6浓度与MS的程度相关,可引起胰岛素抵抗的发生。OBJECTIVE : To explore the changes of levels of tumor necrosis factor - α( TNF - α) and interleukin - 6 ( IL - 6) at different stages of metabolic syndrome (MS) in the elderly patients and the relationship between TNF - α and IL - 6 and insulin resistance (IR). METHODS: Based on the degrees of disorders of glycolipid metabolism, 90 elderly patients with obesity were divided into three groups including MS combined with type 2 diabetes mellitus ( T2DM ) group ( 30 cases ), obesity combined with T2DM group (30 cases) and healthy elderly subjects (control group). Serum concentrations of TNF -α and IL -6 were measured by an enzyme - linked immunosorbent assay (ELISA). RESULTS : ( 1 ) Serum concentrations of TNF - α and IL - 6 were obviously increased among obesity groups compared with control group. Serum levels of TNF - α and IL - 6 were gradually increased among all the groups. There were significant differences among all the groups (P 〈 0. 01 ). (2) Serum concentrations of TNF-α and IL- 6 were correlated positively with the level of HOMA - IR. CONCLUSION : Serum concentrations of TNF - α and IL - 6 in elderly patients with MS are associated with the presence of MS, which induces IR in the elderly.
