关节镜视下治疗膝关节骨陛关节炎的长期疗效观察

摘　　要：目的评价膝关节镜视下手术在骨性关节炎的临床治疗中的效果。方法用膝关节镜对术前诊断为骨关节炎的696例（785膝），常规行关节镜检查和镜下清理术。结果患者全部进行术后随访，随访3个月～4年，Ⅰ级3个月～1年、1～2年、2年以上随访优良率分别为90．8％、84．1％、77．0％；Ⅱ级分别为86．9％、82．1％、74．3％；Ⅲ级分别为43．1％、42．3％、39．7％。结论关节镜检查可以在X线片无改变前，早期诊断膝关节骨性关节炎，镜下清理术具有创伤小、并发症少、患者恢复快等优点，对膝关节早、中期骨性关节炎是一种微创、有效的治疗方法，但对膝关节晚期骨性关节炎的患者疗效不太满意。Objective To evaluate the role of arthroscopy in diagnosis and treatment in knee osteoarthritis. Methods Six hundred and ninety-six patients preoperative diagnosed of knee osteoarchritis received arthroscopy. Results With 3 months to 4 years follow up, level Ⅰ good follow-up rate of 3 months to 1 year, 1-2 years, more than 2 years were 90.8% , 84.1% , 77.0%, respectively; level Ⅱ were86.9%, 82.1%, 74.3% ; level Ⅲ were 43.1%, 42.3% , 39.7%. Conclusions Arthroscopy is a simple and effective therapy for early and middle stage osteoarthritis but doesn＇t show optimistic outcomes in late osteoarthritis.
